BDNF用于抑郁症辅助诊断和疗效判断的初步研究

摘　　要：目的检测脑源性神经营养因子(BDNF)在抑郁症发生发展和治疗中的变化,探讨其用于抑郁症辅助诊断和疗效判断的可行性。方法 42例明确临床诊断抑郁症患者,其中中重型各13、15例,接受6 w常规抗抑郁治疗,治疗前后用酶联免疫吸附法(ELISA)测定血清BDNF水平,并与88例非抑郁症人群比较。结果与对照组比较,轻中重度抑郁症患者血清BDNF水平均有明显下降(P<0.01);且抑郁程度与BDNF水平负相关。治疗后,中重度患者血清BDNF水平均明显升高(P<0.01)。在临床症状显著改善患者,其血清BDNF水平与治疗前比较均呈现不同程度的升高。结论血清BDNF水平可作为临床抑郁症诊断,特别是治疗反应的参考指标。Objective To detect the changes of brain-derived neurotrophic factor（BDNF）in the development and treatment of depression,and to explore its feasibility in the diagnosis and assess of curative effects on depression.Methods Among the 42 patients who were clinically diagnosed depression,13 ones were severe and 15 ones were moderate.They received the conventional antidepressant treatment for 6 w.Before and after the treatment,the serum BDNF levels were measured by the method of enzyme linked immunosorbent assay（ELISA）,and were compared with those of 88 people without depression（control group）.Results Compared with the control group,the serum BDNF level of patients with moderate and severe depression significantly decreased（P0.01）.There was a negative correlation between the degree of depression and the BDNF level.After the treatment,the BDNF levels of patients with moderate and severe depression significantly increased（P0.01）.Among the patients with significant improvement in clinical symptoms,their serum BDNF level increased in various degrees,compared with that before the treatment.Conclusion Serum BDNF level can be regarded as a reference index for the clinical diagnosis of depression and particularly therapeutic reaction.
