选择性深低温断血流对猴脑神经营养因子的影响

摘  要:目的观察猴脑选择性深低温断血流复苏NGF、TrKAQA、GDNF的表达及其对神经营养因子表达的影响。方法 4~10岁健康恒河猴10只,动物随机分为三组:四血管(双侧颈总动脉及椎动脉)阻断冷灌注组(简称四血管组,n=5),两血管(双侧颈总动脉)阻断冷灌注组(简称两血管组,n=3),两血管(双侧颈总动脉)阻断等温灌注组(简称等温灌注组,n=3)。常温下临时阻断双侧颈部血管10 min后,通过一侧颈内动脉冷灌注(或等温灌注),同侧颈内静脉回流,阻断其他颈部血管,建立选择性脑局部体外循环通路,降低脑温至15.1℃±0.9℃,60 min恢复脑血流,实验动物自然复苏。等温灌注组及四血管组实验猴于灌注死亡后和两血管组实验猴饲养到术后12 w处死恒河猴,取脑、固定、脱水、石蜡包埋、切片,用NGF、TrkA、GDNF、Vimentin抗体进行免疫组织化学染色。对额叶恒定视野内NGF、TrkA、GDNF的阳性细胞进行记数并进行统计学分析。结果等温灌注组NGF、TrkA、GDNF有微量表达。四血管组NGF、TrkA、GDNF表达明显上调,与等温组和两血管组比较均有统计学意义(P<0.01)。两血管组NGF、TrkA、GDNF表达较明显上调,与等温灌注组比较有统计学意义(P<0.01)。结论 NGF、TrkA、GDNF表达上调,提示脑选择性超深低温断血流复苏过程中启动了脑自我保护机制,通过内源性神经营养因子表达水平上调,提高了神经细胞对缺血缺氧的耐受性,促进受损脑细胞的修复,是脑缺血的重要保护机制之一。Objective To investigate the efects of the vital organs of selective profound hypothermia and antegrade cerebral perfusion on maca mulattas ischemic brain.Methods eleven 4~10 year-aged macaca mulattas were divided into four-vessel occlusion group(n=5),identical temperature perfusion group(n=3) and two-vessel occlusion group(n=3).12 weeks after operation the macaca mulatta were executed,the brain was immediately removed,followed by fixation,dehydration,embedding with paraffin and section.Immunohistochemical technique was used to determine cellular expression of neurotrophic factors NGF,TrkA,GDNF in respective group.Statistics was analyzed by ANOVA analyses with significance level at P〈0.05.Results The expressions of NGF,TrkA,GDNF protein were significantly lower in identical temperature perfusion groups than those of other groups.The expressions of NGF,TrkA,GDNF protein were higher in four-vessel occlusion group than those in two-vessel occlusion group.Conclusions The expressions of neurotrophic factors are significantly increased of macaca mulatta of resuscitation after selective cerebral profound hypothermia and blood flow occlusion.Selective profound hypothermia may provide protective effects during cerebral ischemia.
