P120和E-cad在乳腺癌中的表达及鉴别诊断意义

摘　　要：目的:探讨P120和E-cad在乳腺浸润性导管癌与浸润性小叶癌中的表达及其鉴别诊断意义。方法:收集海口市人民医院2005年1月～2010年12月间有完整随访资料的浸润性导管癌32例,浸润性小叶癌16例及混合性癌12例。采用SP免疫组化染色检测P120连环蛋白、E-钙黏蛋白和P63抑癌基因表达情况。结果:P120连环蛋白在32例浸润性导管癌中胞膜阳性率为100.0%;在16例浸润性小叶癌中胞浆阳性率为100.0%。E-钙黏蛋白在浸润性导管癌中胞膜阳性率为84.3%(27/32),而在小叶癌中无阳性表达(P<0.01)。P63在浸润性导管癌中有1例阳性其余均为阴性;在小叶癌中3例阳性其余为阴性,二者差异无统计学意义(P>0.05)。P120和E-钙黏蛋白联合应用将12例混合性癌分为浸润性导管癌8例,浸润性小叶癌4例。结论:P120和E-钙黏蛋白联合应用可区分浸润性导管癌和小叶癌,而P63则不能用于二者的鉴别。Objective：To explore the expressions of P120 catenin and E-cadherin in breast invasive ductal carcinoma and invasive lobular carcinoma,and their significances for differential diagnosis.Methods：Thirty-two cases with invasive ductal carcinoma,sixteen cases with invasive lobular carcinoma,and twelve cases with mixed carcinoma who had intact data of follow-up and were treated in the hospital from January 2005 to December 2010 were collected.SP immunohistochemical staining was used to detect the expressions of P120 catenin,E-cadherin,and P63 tumor suppressor gene.Results：The positive rate of P120 catenin in cell membrane of invasive ductal carcinoma group was 100.0%;the positive rate of P120 catenin in cytoplasm of invasive lobular carcinoma group was 100.0%.The positive rate of E-cadherin in cell membrane of invasive ductal carcinoma group was 84.3%（27/32）;E-cadherin didn＇t express in invasive lobular carcinoma group,there was significant difference（P0.01）.In invasive ductal carcinoma group,only one case was found with positive P63;in invasive lobular carcinoma group,three cases were found with positive P63,there was no significant difference（P0.05）.Twelve cases with mixed carcinoma were divided into 8 cases with invasive ductal carcinoma and 4 cases with invasive lobular carcinoma by P120 catenin combined with E-cadherin.Conclusion：P120 catenin combined with E-cadherin can differentiate invasive ductal carcinoma from invasive lobular carcinoma,but P63 doesn＇t have the effect.
