摘　　要：目的:探讨综合干预在降低重性精神患者危险性行为中的作用。方法:选取近5年来在"中央补助地方重性精神疾病管理治疗项目"(简称686)中有危险性行为的重性精神病患者238例,给予综合干预,包括住院期间的药物治疗、心理治疗、行为矫正治疗、工娱治疗等,出院后的社区管理、定期随访、个案管理等干预措施。在干预前后进行病例诊断复核及肇事肇祸危险性评估,并进行统计学处理及相关分析。结果:在686项目开展过程中,经过对患者进行治疗前与治疗后综合干预统计,发现肇事肇祸率明显下降,干预后较干预前差异有高度显著性(干预3个月后比较,t=5.85,P<0.01;干预6个月后,t=4.02,P<0.01;干预12个月后,t=5.03,P<0.01)。结论:综合干预对重性精神疾病患者的危险性行为有明显的降低作用,同时也提高了精神患者对治疗的依从性和家属对重性精神疾病的认识,对精神患者的精神康复具有积极的意义。Objective： To discuss the affect of comprehensive intervention in reducing the risky behaviors of patients with holergasia. Methods： Selected 238 patients with holergasia of risky behaviors in the national holergasia management program（686 projecs） of the last 5years; Provided them comprehensive intervention during hospitalization： such as medications, psychotherapy, behavior modification therapy, occupational and recreational therapy; and during discharged from hospital： such as community management, regular follow-up, case management ect. In order to estimate the affect of comprehensive intervention, Brief Psychiatric Rating Scale （BPRS）, diagnostic review and accident risk assessment were carried out before and after the intervention. The results were dealed with statistics and correlation analysis. Results： Compared with the comprehensive intervention provided before, the accident rate of the patints with holergasia has a significantly difference ： （ 1 ） three months after the intervention （ t = 5.85, P 〈 0. 01 ） ; （2） six months after the intervention （ t = 4. 02, P 〈 0. 01 ） ; （3） twelve months after the intervention （t = 5.03, P 〈 O. O1 ）. Conclusions： Comprehensive intervening had the obvious affect in reducing the risky behaviors of patients with holergasia, and also improved the patients＇ compliance treatment, so it has a positive on the mental rehabilitation of patients.
