摘　　要：目的 探讨蒙特利尔认知评估量表（Montreal Cognitive Assessment,MoCA）在脑出血后认知损害筛查中的应用.方法 应用简易精神状态量表（Mini-Mental State Examination,MMSE）、MoCA对88例脑出血患者和61例健康人进行神经心理评估.行CT或MRI检查,详细收集患者一般资料（包括年龄、性别、受教育程度）及血管性危险因素（vascular risk factor,VRF）如血压、血脂等,将以上神经心理学量表评定结果、血管性危险因素进行对比研究及相关分析.结果 ①MMSE、MoCA总分在脑出血组和对照组都有相关性（P〈0.05）.②脑出血组和对照组MMSE总分均高于MoCA评定总分（P〈0.05）.③MoCA各分项得分中,脑出血组在视空间与执行能力、注意力、语言、延迟回忆4个方面显著低于对照组（P〈0.05）.结论 认知功能障碍是脑出血患者的常见并发症,其中视空间执行能力、注意力、语言及延迟回忆领域受损尤为明显.MoCA对脑出血后认知损害患者的早期筛查较MMSE敏感性、信度更高.Objective To explore the application of Montreal Cognitive Assessment （MoCA） in cognitive impairment after cerebral hemorrhage. Methods 88 cases of cerebral hemorrhage patients and 61 healthy age - matched subjects were selected. All subjects were examined with Mini -Mental State Examination（MMSE） and MoCA, patients and control subjects were examined with CT and MRI. All subjects＇ general data were collected. Results （ 1 ） The total scores of MoCA were correlated with that of MMSE （P 〈 0.05）. （2） The totle scores of MMSE in cerebral hemorrhage and control groups were both higher than the totle scores of MoCA （P 〈 0.05 ）. （3） Some sub - item scores of MoCA in cerebral hemorrhage group were lower than those of the control group such as the scores of Visuopatial/Executive, Attention subitem, Delayed recall and Language （P 〈 0.05 ）. Conclusions Cognitive impairment remains a common complications after cerebral hemorrhage, the Visuopatial/Executive function, Attention subitem, Delayed recall and Language are injured commonly. The sensitivity of MoCA is higher than that of MMSE in the screening of cognitive impairment from patients of cerebral hemorrhage.
