摘 要:目的探讨阿魏酸钠对神经源性痛大鼠脊髓的保护作用及其可能的机制。方法成年雄性Wistar大鼠66只,体重230~250g,随机分为三组:A组(n=6,假手术组):仅暴露坐骨神经而不结扎;B组(n=30,生理盐水组):结扎左侧坐骨神经中段后每天腹腔注入2ml生理盐水,连续3d;C组(n=30,阿魏酸钠组):结扎坐骨神经后向腹腔注入阿魏酸钠100mg·kg-1·d-1(用2ml生理盐水稀释),连续3d。在术前、术后6h、1、3、7及14d分别进行行为学测定,记录缩足阈值,应用免疫组织化学方法和图像分析系统检测脊髓中TNF-α和IL-6的表达,并采用TUNEL法观察脊髓神经元凋亡情况。结果坐骨神经结扎后大鼠可诱发出机械性触诱发痛;TNF-α和IL-6于结扎后6h开始表达,3d达高峰,并于术后14d恢复到术前水平;术后6h脊髓中开始有少量凋亡神经元出现,凋亡指数于术后1d开始迅速增加,术后3d达峰值。与生理盐水组相比较,阿魏酸钠组大鼠脊髓组织中TNF-α和IL-6的表达均显著降低,脊髓神经细胞凋亡指数也显著降低(P<0.05)。结论阿魏酸钠可减轻大鼠疼痛的发展,对脊髓有保护作用,其机制可能与抑制细胞因子的表达,从而抑制神经元凋亡有关。Objective To observe the protective effects of sodium ferulate(SF) on the spinal cord in rats with a modul of neuropathic pain. Methods Sixty-six healthy adult male wistar rats were simply randomized into 3 groups:①sham-operated group(n=6), ②saline group(n=30) and ③ SF group(n=30). The sciatic nerve of each of the rats of group 03 was exposed but not ligated. A model of neuropathic pain was set up in each of the rats of group ②and group ③ by ligating the left sciatic nerve under chloral hydrate anesthesia. The rats of group ② and group ③ were given intraperitoneal injection of normal saline 2 ml and SF 100mg·kg-1·d-1 which was dissolved in 2 ml normal saline respectively after operation. All groups were tested for pain behaviors of the time of paw withdraw by using the von Frey fiber at 6 hour, 1, 3, 7 and 14 days after surgery. Changes of TNF-α and IL-6 expression in the spinal cord were detected through the immunohistochemical methods and image analysis. Apoptosis of spinal cord neurons was dectected with the TUNEL method. Results Mechanical allodynia developed at 6 hour after sciatic nerve ligation; TNF-α, IL-6 and neuronal apoptosis were first detected in spinal cord 6 hour, 1 day and 1 day after injury and thereafter, peaking at 1 day, 3 day and 3 day. The expression of TNF-α and IL-6 in spinal cord and spinal neuronal apoptofic index in group ③ were significantly lower than those in group ② (P〈0.05), and the date of behavioural assessments in group.③ were markedly higher than that in group ② (P〈0.05). Conclusion Sodium ferulate can effectively relieve pain and protect spinal cord in rats with a model of neuropathic pain, which mechanism maybe conclude with depressing the expression of cytokones and consequently inhibiting spinal neuronal apoptosis.
