腮腺浅叶不同区域良性肿瘤功能性切除术的探讨

摘　　要：目的:探讨腮腺浅叶不同区域功能性切除术在腮腺浅叶良性肿瘤治疗中的临床应用。方法:112例腮腺浅叶肿瘤患者术前临床检查配合B超或MRI检查倾向于良性肿瘤,其中72例根据不同区域采用功能性切除术,40例采用传统腮腺浅叶切除术,术后随访1周～5年,观察两组术后并发症及肿瘤复发情况。结果:112例良性肿瘤患者术后肿瘤均无复发,两组在Frey综合征、耳垂区麻木发生率方面差异明显,有统计学意义(P<0.01),两组在面神经功能损伤、涎漏和面部畸形的发生率方面亦有差异,有统计学意义(P<0.05)。结论:腮腺浅叶不同区域功能性切除术疗效优于腮腺浅叶切除术。Objective： To summarize the clinical application of a selective functional tumor dissection in the management of parotid superficial benign tumors.Methods： 112 tumor cases were retrospectively reviewed.Group A（n=72）,tumor dissection was treated with different regional functional excision.Group B（n= 40）,cases were operated via routine surgical modality.The follow-up period was 1 week to 5 years.Postoperative complications and tumour recurrences were compared between these two groups.Results： No postoperative tumour recurrence occurred.There was significant difference（P0.01） between the two groups on the complications of Frey＇s syndrome,postoperative numbness in the auricular lobule.The facial nerve dysfunction,the leak of saliva,facial contour disfigurement were also found statistically different（P0.05） between the two groups.Conclusions： The significance of margin in regional functional excision in the treatment of benign tumors of superficial parotid gland should be considered.
