颅内脂肪瘤的诊断及治疗

摘　　要：目的探讨颅内脂肪瘤的诊断和治疗方法。方法回顾性分析1995～2010年收治的8例颅内脂肪瘤患者的临床资料;2例无明显症状者仅行CT随访;6例有明显症状者行手术治疗,术后行颅脑CT或MRI随访。结果手术的6例中,术后除1例仍有癫痫外(药物可控制),余5例症状缓解或明显改善。8例病人随访1～8年,CT或MRI未发现脂肪瘤复发或残余肿瘤明显增大。结论颅内脂肪瘤多发于胼胝体区,常常合并胼胝体发育不良,CT和MRI有助于确诊本病;对无症状的颅内脂肪瘤患者可密切随访观察,对症状明显者应酌情行显微手术切除肿瘤。Objective To explore the methods to diagnose and treat intracranial lipomas. Methods The clinical data of 8 patients with intracranial lipomas, who were treated from 1995 to 2010, were analyzed retrospectively. Of 8 patients with intracraniaI lipomas, 6 were treated by surgery and followed up by CT or MRI after the surgery and 2 without symptoms were followed up only by CT. Results Of 6 patients with intracranial lipomas undergoing the surgery, 5 were improved in the clinical symptoms and I had epilepsy after the surgery. The following-up by CT or MRI from 1 to 8 years showed that no lipomas recurred or became bigger in all the patients. Conclusions The intracranial lipomas are more common in the corpus callosum region of the brain than those in the other cerehral areas, and were often accompanied with callosal agenesis. Cerebral CT and MRI examination are helpful to making the definite diagnosis Of the intracranial lipomas. The patients without symptoms may receive expectant treatment by followed-up. Microsurgery should be performed in the intracranial lipomas patients with symptoms.
