10例轻链型多发性骨髓瘤的实验室结果分析

摘　　要：目的探讨轻链型多发性骨髓瘤(MM)实验室检查的特点。方法对10例轻链型MM病例的实验室检查结果进行回顾性分析。结果轻链型MM的RBC、HB、UREA、CRE、IgG与IgG型MM比较,差异有显著性意义(P<0.05);轻链型MM的WBC、PLT、IgA、IgM、骨髓浆细胞比例、骨髓原幼浆细胞比例、骨髓成熟浆细胞比例以及溶骨性损害比例与IgG型MM比较,差异无显著性意义(P>0.05)。轻链κ型MM和IgG、κ型MM血清、尿液游离轻链大部分指标比较,差异有显著性意义(P<0.05)。轻链λ型MM和IgG、λ型MM血清、尿液游离轻链各指标比较,差异无显著性意义(P>0.05)。结论轻链型MM的贫血、肾功能损害较突出,完整的免疫球蛋白水平明显低于相应类型的MM。血清、尿液游离轻链定量及血清免疫固定电泳在MM的诊断及分型中起到非常重要的作用。Objective To investigate laboratory characteristics of Light-chain multiple myeloma（LCMM）.Methods To do retrospective analysis on laboratory results of 10 cases of LCMM and compared them with those of IgG type MM statistically.Results There were significant differences in RBC、HB、UREA、CRE、IgG between LCMM and IgG type MM（P〈0.05）.There were no significant differences in WBC、PLT、IgA、IgM、bone marrow total plasma cell ratio、bone marrow immature plasma cell ratio 、 bone marrow matured plasma cell ratio and dissolve osseous injury between LCMM and IgG type MM（P〈0.05）.There were significant differences in most indices of serum and urine free light chain between LCMM-κ type and MM-IgG,κ type（P 0.05）.There were no significant differences in each index of serum and urine free light chain between LCMM-λ type and MM-IgG,λ type（P〈0.05）.Conclusion The extent of anemia and renal dysfunction of LCMM is much more severe than that of IgG type MM.The intact immunoglobulin level is significantly lower than that of corresponding type MM.Serum and urine light chain quantitative determination and serum immunofixation electrophoresis play an important role in diagnosis and type differentiation of MM.
