摘 要:目的 探讨甲状腺癌癌变的机制。方法 应用免疫组化S P法于石蜡切片对 43例甲状腺癌、1 7例腺瘤、1 9例瘤旁甲状腺组织进行了c erbB 2、p53、Bcl 2、c myc、p1 6癌基因产物的检测。结果 癌组织中均有不同程度 1种或 1种以上基因产物表达与腺瘤及瘤旁组组间差异均非常显著 (P <0 .0 1 )。c erbB 2表达随癌组织分化程度增高而增高。p53表达随癌组织分化程度增高而减少 ,与c erbB 2间呈负相关 (P <0 .0 5)。在淋巴结转移病例中c erbB 2阳性率增高明显 (P <0 .0 5)。结论 5种癌基因均分别参与了甲状腺癌的发生发展过程 ,癌变的发生可由多种基因相互作用所致。c erbB 2可能在甲状腺乳头状癌的癌变中起重要作用。癌基因产物检测尤其是同时检测c erbB 2。Objective To study the carcinogenesis.Methods c erbB 2,p53、Bcl 2,c myc and p16 in 43 cases of thyroid carcinoma,17 cases of adenoma and 19 cases of adjacent normal tissues of thyroid tumor were examined in the form of paraffin sections using the method of immunohistochemistry S P.Results In carcinoma tissuse there was one or more than one kind of expression of genes which had highly striking difference to adenoma and normal tissues (P<0.01).The espression of c erbB 2 was increased with increasing degree of differentiation of the carcinoma.The expression of p53 was decreased with the increasing degree of differentiation. A significant inverse correlation was found between p53 and c erbB 2(P<0.05),The positive rate of c erbB 2 was obviously higher in the cases of lymphaden translation(P<0.05).Conclusions The result suggested that five oncogene proteins were involved in the occurrence and development of thyroid carcinoma. The occurrence of thyroid carcinoma may result from the interaction of some oncogenes.c erbB 2 may play an important role in papillary carcinoma.Oncogene proteins were assayed,esp.c erbB 2 and p53 were examined at the same time,to provide clinical pathological diagnosis,gence treatment and prognosis of thyroid carcinoma with reference indexes.
