肠道支架置入治疗结直肠恶性梗阻的疗效观察

摘　　要：目的探讨经结肠镜及X线下置入钛镍记忆合金支架治疗结直肠恶性梗阻的安全性与临床疗效。方法选择19例结直肠癌患者作为实验组,其中5例结直肠恶性梗阻并有手术指征的患者接受临时支架置入术;14例无手术指征的结直肠恶性梗阻患者接受永久性支架置入术姑息治疗。选择同期外科19例结直肠癌并梗阻的患者作为对照组,观察治疗组支架植入成功率和梗阻改善情况及两组患者生活质量及生存时间。结果与对照组结果相比,实验组支架放置成功率为100%,肠道梗阻均有缓解,生活质量明显改善,而平均生存时间相当,差异无显著性(P>0.05)。结论结肠镜及X线下置入钛镍记忆合金支架治疗结直肠癌恶性梗阻是一种简单可行、安全有效、并发症少的临时性或永久姑息性治疗手段,能减轻患者痛苦、提高生活质量、与外科手术的平均生存时间相当。[Objective] To evaluate the clinical efficacy and safety of endoscopic metallic stent for treating malignant colorectal obstruction by endoscopy and X-ray.[Methods] 19 cases malignant colorectal carcinoma were selected as stent experiment group,5 cases malignant colorectal obstruction with operation indications accepted temporary stenting;14 cases without operation indications accepted stenting,as is permanent treatment.At the same period,19 cases malignant colorectal obstruction in surgery were selected as operation treatment group.To observe the achievement rate of the stent and the improvement condition of obstruction of stent treatment group and the life quality and live time of two groups.[Results] Compared the operation treatment groups,the achievement rates of the stent were 100% in experiment group,the obstruction had evidently improvement,and the quality of life had significantly improvement,however,average survival time hadn＇t different significantly（P 0.05）.[Conclusion] Endoscopic metallic stent treatment for malignant colorectal obstruction by endoscopy and X-ray is a safe,effective and well tolerated method for the temporary remission or permanent treatment.It can reduce trauma,raise quality of life and have similar average survival time with surgery in advanced cancer patients.
