口腔内热水刺激反应的功能性磁共振成像研究

摘　　要：目的:探讨口腔热水刺激时的大脑皮层区域定位。方法:选取13名口腔状况健康的右利手志愿者,56℃热水刺激口腔,并以室温水作为对照。采用口腔热刺激过程(热刺激信号减静息信号)的组块设计方法,采集功能性磁共振成像扫描数据并用SPM99软件包进行结果的数据分析。结果:组分析结果显示激活区:左侧丘脑(-6,-11,17)和运动语言区(BA44),口腔躯体感觉运动皮层(45,-8,14)、双侧运动后区、双侧顶下小叶(BA40);岛叶、扣带回前部(anterior cingulate cortex,ACC)(-15,8,44)、顶上小叶。右侧顶叶的楔前叶和纹状体激活;岛叶和口腔躯体感觉皮层激活明显T值45.26,两组fMRI横断面的融和图比较,在口腔感觉运动区、岛叶、ACC重叠。结论:口腔内热水明显激活左侧丘脑和躯体感觉运动皮层,表明口腔内热刺激可以激活大脑皮层的相关区域。Objective： To evaluate the feasibility of fMRI on the location of brain regions related to hot stimulated withwater. Methods： Thirteen healthy right-handed volunteers were selected to perform the fMRI study and were divided into two groups randomly. The subjects were served with 56℃deionized hot water in experimental group, and subjects were served with 23℃ warm water in control group. Block designed tMRI scan covering the whole brain was carried out. The fused cross-sectional maps of the two groups were compared. Results： The group showed robust activations in left thalamus （-6, -11, 17）, bilateral activations insular, pre/post-central gyrus oral sensory/motor（45, -8, 14）. Signals in motor language area （BA44）, BA6, parietal lobule （BA40, 7） were strong. BOLD signals were also found unilaterally in right frontal Lobe, striatum and precuneus. Pre/post-central gyrus （oral sensory/motor）, ACC（-15, 8, 44） and insular/operculum were overlapping activated in both groups. Conclusions： The magnitude and volume of activation was significantly different between hot and warm stimulation intra-orally. It indicates that specific functional regions in the human brain cortex could be acti- vated by intra-oral hot stimulation in fMRI study.
