外伤性视网膜脱离的预后影响因素分析

摘　　要：目的:对眼外伤致视网膜脱离患者进行回顾性研究,探讨外伤性视网膜脱离玻璃体手术治疗的预后影响因素。方法:收集在我科经玻璃体手术治疗的外伤性视网膜脱离患者121例,记录患者资料:年龄、性别、受累眼、眼外伤类型、视网膜脱离类型、术前与术后视力、术前合并症、术后并发症等,并进行资料分析。结果:121例患者(121只眼),术后成功修复率分别为:钝挫伤82.21%、穿通伤56.32%、眼内异物伤61.28%、破裂伤41.73%。术前合并玻璃体积血者成功修复率为55%(P<0.01),术前合并增殖性玻璃体视网膜病变者成功修复率43.47%(P<0.01)。术前合并视网膜嵌顿的成功修复率46.66%(P<0.05)。结论:外伤性视网膜脱离男性是高发人群,玻璃体手术在视网膜复位中是有效的。钝挫伤较其他类型眼外伤引起的视网膜脱离预后较好。术前合并玻璃体积血、增殖性玻璃体视网膜病变、视网膜嵌顿者成功修复率较低。对预后较差的开放性眼伤引起的视网膜脱离应防止术后PVR的发生,以减少视网膜再脱离。Objective：The aim of this study was to carry on retrospective study in exploring the influencing factors of prognosis in traumatic retinal detachment managed with vitrectomy. Methods： This retrospective study included 121 patients with retinal detachment caused by ocular trauma. Data on patient presentation： age, sex, involved eye, types of injury and retinal detachment, preoperative and postoperative visual acuity, preoperative and postoperative complications and operative filler were collected. Statistics analysis was used. Results：A total of 121 patients （121eyes） were included in the study. Successful retinal re - attachment rate was achieved in 82. 21% of contusion, followed by, penetrating 56.32% , IOFB 61.28%, rupure 41.73% The repair rate is55 % （P 〈 0.01） in complicating with intravitreous hemorrhage, 43.47 % （ P 〈 0.01 ） in with traumatic proliferative vitreoretinopathy, 48.89 % （P〈 0.05） in with retinal incarceration. Conclusion： The male is at particular risk. Vitrectomy appears to give an effective reattachment rate for traumatic retinal detachment. On the prog- nosis, retinal detachment caused by contusion is better than other types of injuries. The repair rate is lower in preoperative complications, such as vitreous hemorrhage, PVR and retinal incarceration. We should pay attention to preventing PVR after operation.
