微创血肿穿刺清除术治疗高血压脑出血效果分析

摘　　要：目的探讨颅内微创血肿清除术治疗高血压脑出血的疗效和并发症情况。方法选择本院2007年1月至2009年1月应用微创血肿穿刺清除术治疗高血压脑出血45例(微创术组),与2005年1月至2006年12月未行血肿穿刺进行内科保守治疗的42例患者(内科治疗组)进行对比分析。以治疗6个月后日常生活能力(ADL)和病死率以及治疗30 d发生并发症作为判断疗效的指标,比较两组的疗效。结果微创血肿清除术组ADL优于内科治疗组,两组比较差异有统计学意义(U=6.303,P=0.000);微创血肿清除术组患者发生严重并发症情况明显低于内科治疗组,两组比较差异有统计学意义(χ2=7.1314,P=0.008);治疗6个月内科治疗组和微创血肿清除术组死亡率分别为42.9%和22.2%,其差异有统计学意义(χ2=4.2382,P=0.04))。结论微创血肿清除术治疗高血压脑出血安全有效。Objective To explore the therapeutic efficacy and complication incidence of minimally invasive craniopuncture therapy in treatment of hypertension intracerebral hemorrhage.Methods Totally 45 patients with hypertension intracerebral hemorrhage receiving minimally invasive craniopuncture therapy plus urokinase therapy between January,2007 and January,2009,were recruited as the cases.42 patients treated with conservative control therapy between January,2005 and December,2006,was recruited as the controls.Several parameters were evaluated including patients＇complication incidence 30d after treatment,activities of daily living（ADL） and mortality within 6 months.Results The ADL in mini-traumatic operation group were significantly superior to those in the medical treatment group（U=6.303,P=0.000）;in the mini-traumatic operation group,the complication incidence was significantly lower than that of the control group（χ2=7.131,P=0.008）;The mortality in mini-traumatic operation group was 22.2%,which was lower than 42.9%in the control group in 6 months after treatment（χ2=4.238,P=0.04）.Conclusions Minimally invasive craniopuncture therapy is more effective than medical treatment alone.It is an effective treatment method for the patients with hypertension intracerebral hemorrhage.
