2型糖尿病家系胰岛素抵抗与血清抵抗素水平的相关性研究

摘　　要：目的了解2型糖尿病家系人群胰岛素抵抗与血清抵抗素水平的相关性,初步阐述抵抗素在胰岛素抵抗发病机制中的作用。方法检测2型糖尿病家系一级亲属89例(实验组)的血清抵抗素、血脂、胰岛素等水平,并与血糖正常的非糖尿病家系者(对照组)进行对比分析,考察其血清抵抗素水平变化与胰岛素抵抗的相关性。结果 2型糖尿病家系一级亲属FINS、2h INS、TG、血清抵抗素、HOMA-IR的水平显著高于对照组(P<0.01),一级亲属的HDL-C水平显著低于对照组(P<0.05),两组的FPG、2h PG、TC、LDL-C水平的比较无显著性差异(P>0.05)。血清抵抗素与BMI、FINS、2h INS、TC、HOMA-IR呈正相关(P<0.01),与2 h PG呈负相关(P<0.05),与FPG、TG、HDL-C、LDL-C无显著相关性(P>0.05)。BMI、FPG、2 h PG、血清抵抗素为2型糖尿病家系一级亲属胰岛素抵抗的危险因素。结论 2型糖尿病家系血糖正常的一级亲属存在显著的胰岛素抵抗,其抵抗素对胰岛素抵抗程度有明显的影响。[ Objectives ] To find out the insulin resistance of type 2 diabetes families and their correlation with the serum resistin levels, and to describe the resistance role of resistin in the pathogenesis of insulin. [ Methods ] 89 first degree relatives of type 2 diabetes families （experimental group） were selected and their serum resistin, lipids and insulin levels were detected, and compared with normal blood glucose in non-diabetic families （con!rol group）, the correlation of serum resistin levels and insulin resistance was analyzed. [ Results ] Type 2 diabetes families firstdegree relatives FINS, 2h INS, TG, serum resistin, HOMA-IR levels were significantly higher （P 〈0.01）, first-degree relatives HDL-C level was significantly lower than the control group （P 〈0.05）, the FPG, 2h PG, TC, LDL-C levels showed no significant difference （P 〉0.05）. Serum resistin and BMI, FINS, 2h INS, TC, HOMA-IR were positively cor- related （P 〈0.01）, serum resistin and 2h PG was negatively correlated （P 〈0.05）, serum resistin and FPG, TG, HDL-C, LDL-C was no significant correlation （P 〉0.05）. BMI, FPG, 2h PG, serum resistin levels were the risk factors for insulin resistance of family relatives of type 2 diabetes families. [ Conclusions ] The first-degree relatives of type 2 diabetes families have significant insulin resistance, resistin has a significant impact with their insulin resistance.
