摘 要:目的探讨α-平滑肌肌动蛋白(α-SMA)在肝癌中的表达及与肝癌侵袭转移的关系。方法收集非肝癌的肝脏组织15例,肝癌并伴门静脉转移的癌组织30例,按病理分化程度分为3组:低分化组、中分化组和高分化组。采取逆转录-聚合酶链反应(RT-PCR)检测肝癌组织中α-SMAmRNA的表达和免疫组织化学的方法检测理.SMA在癌组织及门静脉癌栓组织的表达。结果RT-PCR结果显示0L.SMA在所有肝癌组织中均呈阳性表达;免疫组织化学显示α-SMA在对照肝脏、肝癌组织和门脉癌栓组织的表达值分别为125.69±29.17、187.43±17.66、218.70±13.52,方差分析显示α-SMA表达差异有统计学意义(P〈0.01),门脉癌栓组表达最高;按照肝脏病理分为3组:低分化组、中分化组和高分化组,表达值分别为237.14±25.83、190.57±19.90、165.67±22.64,方差分析显示α-SMA表达差异有统计学意义(P〈0.01),在低分化组表达最高。结论α-SMA在肝癌中表达明显增加,特别是门静脉癌栓组及低分化组,与肝癌的侵袭、转移有关。Objective ;to study on the expression of a-smooth muscle actin (α-SMA) in liver cancer and its relation with the invasion and metastasis of liver cancer. Methods Collect liver tissues ineluding no liver cancer tissues, liver cancer and with metastasis in portal vein; divide them into three groups in accordance with the pathologic differentiation degree: low differentiation group, medium differen- tiation group and high differentiation group. Use reverse transcription-polymerase chain reaction (RT-PCR) to test the expression of α-SMA mRNA in the tissues of liver cancer, and use the method of immunohisto- chemistry to test expression of α-SMA in the cancer tissue and the tumor thrombus tissues in portal vein. Results The result of RT-PCR shows that α-SMA expression is positive in all the tissues of liver cancer; the result of immunohistochemistry shows that the expression values of the control liver tissues, tissues with liver cancer and tumor thrombus tissues in portal vein are 125.69±29. 17, 187. 43 ± 17. 66 and 218.70 ± 13. 52 respectively, Variance analysis shows that the difference of α-SMA expression has statistically significance (P 〈 0. 01 ), of which, tumor thrombus tissues in portal vein have the highest expression value ; the expression values of low differentiation group, medium differentiation group and high differentiation group are 237.14 ± 25.83, 190.57 ± 19. 90 and 165.67 ±22. 64 respectively. The variance analysis shows that the difference of α-SMA expression has statistically significance ( P 〈 0. 01 ), which is the highest in the low differentiation group. Conclusion There is a significant increase expression of α-SMA in liver cancer, especially in the group of tumor thrombus tissues in portal vein and the low differentiation group. It is related to the invasion and metastasis of liver cancer.
