卵巢巧克力囊肿低场MRI诊断

摘　　要：目的探讨卵巢巧克力囊肿的低场MRI(磁共振成像)诊断价值。方法收集12例卵巢巧克力囊肿的低场MRI资料,进行回顾性分析,所有病例均经手术病理证实。结果 12例卵巢巧克力囊肿共15个病灶均为囊性,2例病灶内有壁结节。T1WI呈高、稍高信号。T2WI信号差异较大,呈高信号、稍高信号、等信号或高低混杂信号。其中有10个病灶在STIR(短时反转恢复)脂肪抑制序列出现信号抑制征象。结论卵巢巧克力囊肿信号表现复杂,在低场MRI上盆腔囊性占位中观察到短T1、长T2信号,STIR出现信号抑制现象时,并不能就此鉴别脂肪与出血成分。综合分析病灶内部结构,增强表现以及临床特点,有利于鉴别诊断。Objective To investigate the low-field MRI diagnosis value of Chocolate cyst.Methods The Low-field MRI appearances of 12 cases of Chocolate cyst confirmed by operation and pathology were analyzed retrospectively.Results 15 chocolate cyst of ovary are cystic masses in 12 patients;2 cases with mural nodules inside.The signal of lesions are high in T1WI,mixed signal in T2WI;and the signal is suppressed in STIR sequence in 10 lesions.Conclusion The MRI signal of chocolate cyst of ovary is complicated.Identification of bloody and fat ingredients cannot be done when the cystic masses in pelvic with short T1、long T2、signal suppression in STIR sequence were observed.The differential diagnosis of chocolate cyst of ovary can be decided,with the internal composition、the performance of enhanced examination and clinical characteristic comprehensively analyzed.
