摘 要:探讨在进行性压迫性脊髓损伤过程中白质纤维溃变的规律。方法:采用自行设计的压迫装置制作进行性压迫性脊髓损伤模型。运用H-E、Luxol fast blue(LFB)、透射电镜和免疫荧光等方法,分别于压迫后1、3、7d观察脊髓白质纤维变化。结果:脊髓受压1ct后,白质髓鞘化神经纤维出现水肿,排列疏松、髓鞘缺失等退行性溃变;随着压迫时间延长,神经纤维溃变加重,纤维数目逐渐减少,与对照组和正常组比较差异有统计学意义。髓鞘碱性蛋白阳性神经纤维变性,数量减少。结论:进行性压迫性脊髓损伤可诱发神经纤维脱髓鞘病变,并随着压迫时间推移溃变逐渐加重。To observe the changes of nerve fiber degeneration of the white matter in the progressive compression spinal cord injury (PCSCI). Methods: Forty-two adult Sprague-Dawley rats were randomly divided into 3 groups: a normal group (n=6), an operation group (n= 18) and a sham operation control group (n= 18). The PCSCI model was established with a self-made device in the operation group. The operation process in the sham operation control group was the same as the operation group except the compression of the spinal cord. The segments of compression spinal cord were properly taken at an interval of 1, 3 and 7 days after operation. HE, Luxol fast blue (LFB) ,electron microscope (EM) and immunofluores- cence (IF) staining were performed to observe the pathological changes of the white matter. Results: HE staining showed the edema and spongy degeneration to different degrees in the white matter of the injured spinal cord at 1, 3 and 7 days after compression. EM showed that the myelin sheath splitted after compression. LFB staining showed that 1 day after compres- sion the nerve fibers became edema, the extracellular space became wider and the degeneration of nerve fiber occurred in the white matter; 3 days after compression, the nerve fiber degeneration was much more severe in the white matter; 7 days after compression, a great number of fibers were lost. Compared with the normal and sham operation groups, the number of my- elinated fibers was markedly decreased in compressed segments of the model group. IF showed that myelin basic protein-pos- itive fibers were degenerated and lost, coinciding with LFB staining. Conclusion: Compressed spinal cord injury may induce nerve fibers demyelination in the white matter and the demyelination occurs time-dependently.
