胃肠间质瘤螺旋CT征象与基质金属蛋白酶-9及乙酰肝素酶表达的相关性研究

摘　　要：目的探讨胃肠间质瘤的CT征象与基质金属蛋白酶-9(MMP-9)及乙酰肝素酶(Hpa)表达的相关性。方法回顾性分析54例经手术病理证实为胃肠间质瘤的CT表现特点,并与肿瘤组织MMP-9及Hpa表达做对照分析。结果肿块呈类圆形或椭圆形30例,分叶状24例,MMP-9及Hpa阳性表达与GIST的的恶性程度(阳性表达率分别为51.4%和84.2%、48.6%和78.9%)肿瘤边界(阳性表达率分别为48.1%和77.8%、37.0%和81.5%)、肿块大小(阳性表达率分别为46.7%和83.3%、40.0%和83.3%)、肿瘤中心有无坏死(阳性表达率分别为86.7%和53.8%、86.7%和48.75%)、有无转移有明显关系(阳性表达率分别为93.3%和51.3%、93.3%和62.1%)(P<0.05)。结论对肿瘤的形态、定性及远处转移诊断,螺旋CT与MMP-9及Hpa表达有很好的一致性,螺旋CT的诊断结果准确可信;螺旋CT征象与MMP-9及Hpa表达相结合能提高GIST侵袭、转移和预后评估的准确性。Objective To discuss the CT features of gastric stromal tumors（GSTs）and their correlation with matrix metalloproteinase-9（MMP-9） and heparanase（Hpa）. Methods The CT features and the expression of MMP-9 and Hpa of 56 patients with histology proven GISTs were analyzed retrospectively. Results MMP-9 and Hpa expression in GIST had correlation with tumor size （〈5era and 〉 5cm）, tumor border（clear and unclear）, whether or not necrosis in the center, and whether or not metastasis. There were statistical significant difference （P〈0.05）. Conclusion The results of spiral CT was credible and accurate. Spiral CT features combining with the expression of MMP-9 and Hpa can improve accuracy of invasion, metastasis and prognosis assessment of GIST.
