摘 要:目的探讨紧密连接蛋白Claudin-1、Claudin-4和Caspase-3在结直肠癌组织中的表达及临床意义。方法应用免疫组化SP法,检测60例结直肠癌和20例正常结直肠粘膜组织中紧密连接蛋白Claudin-1、Claudin-4和Caspase-3的表达水平。结果 Claudin-1、Claudin-4在结直肠癌组中阳性表达率分别为76.6%、85.0%,显著高于正常组的20.0%、30.0%(P<0.01);而Caspase-3在结直肠癌组中表达阳性率为21.7%,显著低于正常组的60.0%;有淋巴结转移组Claudin-1、Claudin-4的表达阳性率分别为50.0%、55.0%,明显高于无淋巴结转移组26.6%、30.0%(P<0.05);Caspase-3在有、无淋巴结转移组阳性率分别为10.0%、15.5%(P>0.05);TNM分期一、二组间Claudin-1、Claudin-4和Caspase-3蛋白表达阳性率差异有统计学意义(P<0.05),而其它组间阳性表达率差异无统计学意义。结论 Claudin-1、Claudin-4的高表达在结直肠癌的发生发展过程中可能起着促进作用,其表达与淋巴结转移、TNM分期有关,Caspase-3可作为结直肠癌预后标志物。Objective To explore expression of Claudin-1,Claudin-4 and correlation study in colorectal cancer(cRc).Method Immunohistochemical staining detected expression levels of tight junction protein Claudin-1,Claudin-4 and Caspase-3 in 60 cases of cRc and 20 normal colorectal mucosa(ncrm).Results Positive expression rates of Claudin-1,Claudin-4 in cRc were 76.6% and 85%,significantly higher than that the positive expression rates were 20% and 30% in ncrm tissues(P0.01);And Caspase-3 in cRc tissues and the positive rate was 21.7%,significantly lower than that of ncrm tissue expression positive rate 60%;Lymphnode metastasis group Claudin-1,Claudin-4 expression positive rate was 50%,55%,obviously higher than that without lymph node metastasis group expression positive rate was 26.6%,30%(P〈0.05);Caspase-3 in,no lymph node metastasis group the positive rate was 15.0%,10.0%,no significant difference of P〈0.05.TNM stage between the three and one groups Claudin-1,Claudin-4 and caspase-3 protein expression positive rate had a significant difference at P〉0.05,While the other groups had no significant difference between the positive expression rate.Conclusion The high expression of the Claudin-1,Claudin-4 in cRc development and progression may play a facilitating role,and relate its expression with lymph node metastasis,TNM stage,caspase-3 may become prognosis markers of colorectal cancer.
