^(18)F-FDG PET-CT延迟显像对肺部炎性假瘤诊断价值的研究

摘　　要：目的探讨18F-FDG PET-CT延迟显像对肺部炎性假瘤诊断价值。方法回顾我科自2006年1月至2011年1月,经手术病理证实为炎性假瘤42例患者,在手术前经PET-CT全身常规和注射后3 h肺部延迟显像检查,测定两次显像病灶的标准摄取值(SUVmax),并计算18F-FDG的储留指数(RI)。18F-FDG PET-CT诊断炎性假瘤预测性标准:(1)1.4>SUVmax>0.5;(2)1.25 mm薄层CT图像形态学征象。18F-FDG PET/CT诊断炎性假瘤回顾性标准:(1)1.4>常规SUVmax>0.5,18F-FDGRI<5%;(2)2.5≥常规SUVmax≥1.4,18F-FDG RI<8;(3)4≥常规SUVmax≥2.5,18 F-FDG RI<10;(4)常规SUVmax>4,18F-FDG RI<15;(5)1.25 mm薄层CT图像形态学征象。把应用以上两种诊断标准做出的诊断结果相比较。结果应用预测性诊断标准,22例PET/CT影像诊断结果与病理相符合,8例被误诊断为结核球,12例被误诊断为肺癌。诊断正确率为52.3%。应用回顾性诊断标准,34例PET/CT影像诊断结果与病理相符合,1例被误诊为结核球,7例被误诊为肺癌。诊断正确率为80.9%。结论 18F-FDG PET-CT延迟显像有助于对炎性假瘤的检出,结合应用SUVmax及RI两项指标可提高诊断正确率。Objective To evaluate the diagnostic value of 18F-FDG PET-CT in differentiating pulmonary inflammatory pseudotumor.Methods From January 2006 to January 2011,42 patients were diagnosed pulmonary inflammatory pseudotumor by clinical diagnosis and pathology.All patients underwent 18F-FDG PET-CT early and 4h delayed imaging after intravenous injection of FDG.The standardized uptake value（SUV） was calculated for both time points and retention index（RI） was also calculated.predictive diagnostic criteria of18F-FDG PET-CT in differentiating pulmonary inflammatory pseudotumor：（1）1.4SUVmax0.5;（2）the morphological imformation provided by thin planer scanning with CT（planer thickness 1.25 mm）.retrospect diagnostic criteria of18F-FDG PET-CT in differentiating pulmonary inflammatory pseudotumor：（1）1.4常规SUVmax 0.5,18F-FDG retention index（RI）5%;（2）2.5≥常规SUVmax≥1.4,18F-FDG retention index（RI）8;（3）4≥常规SUVmax≥2.5,18F-FDG retention index（RI）10;（4）常规 SUVmax4,18F-FDG retention index（RI）15;（5） the morphological imformation provided by thin planer scanning with CT（planer thickness 1.25 mm）.Results Apply predictive diagnostic criteria,The number of patients with pulmonary inflammatory pseudotumor,tuberculoma,lung cancer were diagnosed is 22,8,12 cases respectively.accuracy of diagnosis is 52.3%.Apply retrospect diagnostic criteria,the number of patients with pulmonary inflammatory pseudotumor,tuberculoma,lung cancer were diagnosed is 34,1,7 cases respectively and these 7 cases of lung cancer were misdignosis.accuracy of diagnosis is 80.9%.Conclusion 18F-FDG PET-CT delayed imaging is a reliable imaging protocol for differentiating pulmonary inflammatory pseudotumor.delayed imaging combining SUVmax and RI can improve the accuracy of diagnosis.
