三维适形放疗同步化疗治疗局限期小细胞肺癌的临床研究

摘　　要：目的观察三维适形放疗(3D-CRT)同步化疗对局限期小细胞肺癌(LSCLC)的疗效及毒副反应。方法 69例LSCLC患者采用顺铂+足叶乙甙(PE)或卡铂+足叶乙甙(CE)方案化疗同步三维适形放疗,然后继续化疗,共4～6周期,观察疗效及毒副反应。结果完全缓解32例,总有效率为85.5%,1、2、3年总生存率分别为72%、35%、12%。全组未观察到4级急性和晚期副反应。结论 3D-CRT同步化疗用于LSCLC可获得满意的近、远期疗效,毒副反应小,有较高的临床可行性。Objective To evaluate the thrapeutic effects and compilcations of three-dimensional conformal radiotherapy(3D-CRT)and concurrent chemotherapy in patients with limited stage small cell lung cancer(LSCLC).Methods 69 histologically proved LSCLC patients received chemotherapy of PE or CE regimen and concurrent 3DCRT and then received consolidate chemotherapy for four to six cycles.To evaluate therapeutic effects and complications.Results 32 patients had complete response and the overall response rate was 85.5%.The 1-,2-and 3-year survival rates were 72%,35% and 12%.The median survival time was 14.5 months.No grade 4 acute and late toxicities were observed.Conclusions In the treatment of LSCLC,3D-CRT and concurrent chemotherapy can achieve satisfactory short-term and long-term outcomes with acceptable complications.
