摘  要:目的探讨CD44在重症急性胰腺炎(severe acute pancreastitis,SAP)时胰腺及肺损害机制中的作用,并观察中药清胰Ⅱ号颗粒剂对CD44的影响并探讨其机理。方法将SD大鼠36只随机等分为三组:假手术组(A组)、SAP组(B组)、治疗组(C组)。以30 g/L牛磺胆酸钠逆行注入胰胆管制作大鼠SAP模型。C组以250 g/L清胰Ⅱ号颗粒剂灌胃给药(10 mL/kg),6小时1次,A组、B组用同等剂量生理盐水以相同方式灌胃。制模后24小时取材,应用免疫组织化方法检测胰腺、肺组织中CD44的表达,同时观察胰腺、肺脏组织病理改变;测定肺的湿重/干重比(W/D)。结果 SAP组胰腺及肺脏组织损伤明显,CD44的表达呈阳性,血清淀粉酶活性、肺的湿重/干重比(W/D)显著升高,与假手术组、治疗组比较有统计学意义(P<0.01、P<0.05);治疗组胰腺及肺脏组织中CD44的表达减弱,血清淀粉酶活性、肺的湿重/干重比(W/D)降低,与SAP组比较有统计学意义(P<0.05)。结论 CD44在SAP模型大鼠肺损伤中可能具有重要作用,中药清胰Ⅱ号颗粒可抑制胰腺、肺组织CD44的表达,减轻SAP时胰腺和肺损害,对其有保护作用。Objective To observe the CD44 expression in the pancreas and lungs and explore the mechanism of Chinese medicine Qing Yi II on treating rats followed sodium taurocholate-induced severe acute pancreastitis(SAP).Methods 36 healthy SD rats were randomly divided into three groups:the sham-operated group(Group A),SAP model group(Group B)and the treated group(Group C),Rats in Group B and C were into SAP model by retrograde injection of 30 g/L sodium taurocholate into bilio-pancreatic duct.Qing Yi II Granules was administered,beginning from 1 hour after modeling for three times(every 6 hours)per day via intragastric in fusion to roupC in dose of 10 ml/kg(250 g/L).While to the other two groups,equal volume of saline was infused instead.All animals were sacrificed after modeling.The pancreas and lungs were teken and the CD44 expression of tissue were examined.Pathologic change of pancreas、lungs tissues were observed under microscope and examined the lung wet/dry weight.Results The expression of tissue CD44 were higher in Group B than that in Group A(P0.01),and lower in Group C than that in group B(P0.05),Group B had serious pathologic damage in pancreas and lungs.Conclusions The expression of CD44 might play an important role in organ dysfunction of the pancreas and lungs in SAP rats.Chinese medicine Qing Yi II Granules could depress the expression of tissue CD44 and alleviate the pathologic damage of the pancreas and lungs in rats with acute necrotizing pancreatitis-associated organ injury.
