输尿管转移癌的诊治

摘　　要：目的探讨输尿管转移癌的诊断、治疗的方法和疗效。方法将2007年8月～2009年12月收治的输尿管转移癌患者1例。经胃镜、输尿管镜检查确诊为胃癌输尿管转移,给予放置双J管、肾穿刺造瘘等措施解除输尿管梗阻、缓解肾积水、改善肾功能。并配合化疗、免疫治疗、中药治疗以及其他对症治疗延长生存时间、减轻痛苦。结果留置双J管初期肾积水减轻,1月后肾积水再次加重,行肾穿刺造瘘引流。术后11月死亡。结论胃癌发生输尿管转移较少见,多数已无手术条件,可行输尿管镜留置双J管、肾造瘘治疗,配合化疗、免疫治疗、中药治疗等措施减轻痛苦、延长生存时间。Objective To investigate ureteral metastasis of cancer diagnosis,treatment methods and curative effect.Methods From 2007 August to 2009 December were treated 2 cases of ureteral metastasis cancer patients.After the gastroscope,ureteroscopy for ureteral metastasis from gastric cancer diagnosis,giving the place double J tube,nephrostomy and other measures to lift ureteral obstruction,hydronephrosis,renal function improved remission.Combined with chemotherapy,immune therapy,Chinese medicine treatment as well as other symptomatic treatment prolongs survival time,relieve pain.The results of indwelling double J tube early hydronephrosis loss,respectively,in January February again increased after hydronephrosis,renal puncture fistulation drainage.1 cases of postoperative death in June,1 cases of postoperative death in November.Conclusion Gastric carcinoma ureteral metastasis is rare,most have no operation conditions,feasible ureteroscopy indwelling double J tube,renal fistula treatment,combined with chemotherapy,immune therapy,Chinese medicine treatment measures to relieve pain,prolong the survival time.
