近视眼LASIK术后调节灵洁度变化与视觉疲劳的关系

摘　　要：目的研究近视眼LASIK术后调节灵活度(accommodative facility,AF)的变化情况以及与视觉疲劳关系。方法 32例(64眼)近视患者按年龄分成30岁以下组和30岁以上组,行标准LASIK手术,术前及术后1周、1个月分别测量双眼AF,术后1个月对术后视觉疲劳症状情况行问卷调查,采用配对t检验比较手术前后AF的变化,用Logistic回归分析法分析手术前后AF、年龄以及屈光度等对术后视觉疲劳的影响。结果 30岁以下组术前双眼AF为(7.12±2.02)c·min-1,术后1周为(6.22±1.73)c·min-1,与术前相比差异无统计学意义(P>0.05);术后1个月为(9.46±1.97)c·min-1,与术前相比差异有统计学意义(P<0.05)。30岁以上组术后1周AF由术前(5.68±2.51)c·min-1显著降低至(4.02±1.59)c·min-1,差异有统计学意义(P<0.05),术后1个月恢复至(6.03±1.61)c·min-1,与术前相比差异无统计学意义(P>0.05)。问卷调查发现,术后视觉疲劳症状阳性者13例(40.6%),阴性者19例(59.4%);回归分析表明,患者术前及术后1周AF、年龄以及术前等效球镜度数与视觉疲劳症状显著相关。结论近视眼LASIK术后早期AF下降,以后逐渐恢复,可能是术后视觉疲劳的重要原因之一。Objective To investigate the changes of accommodative facility（AF） in myopia after laser in situ keratomileusis（LASIK） and its relation with asthenopia.Methods Thirty-two myopic patients（64 eyes） who underwent standard LASIK were divided into two groups by the age,30 years and ≥30 years.AF was measured binocularly before LASIK,1 week and 1 month after LASIK.Asthenopic symptoms were surveyed by the questionaire at 1 month after LASIK.AF at pre-operation and post-operation was compared using paired t test.The effects of preoperative and postoperative AF,age and refractive spherical equivalent on asthenopic symptom were analyzed by Logistic regression.Results For the group 30 years,AF of pre-operation,postoperative 1 week and 1 month were（7.12±2.02）c·min-1,（6.22±1.73）c·min-1 and（9.46±1.97）c·min-1,respectively,there was no statistical difference between pre-operation and postoperative 1 week（P0.05）,but statistical difference between pre-operation and postoperative 1 month（P0.05）.For the group ≥30 years,AF of pre-operation,postoperative 1 week and 1 month were（5.68±2.51）c·min-1,（4.02±1.59）c·min-1 and（6.03±1.61）c·min-1,respectively,there was statistical difference between pre-operation and postoperative 1 week（P0.05）,but no statistical difference between pre-operation and postoperative 1 month（P0.05）.There were 13 cases（40.6%） with positive asthenopic symptom and 19 cases（59.4%） with negative asthenopic symptom by the questionaire.Logistic regression showed that age,preoperative spherical equivalent and AF at pre-operation and postoperative 1 week were positive related with asthenopia.Conclusion AF in myopia may decrease in the early stage after LASIK and gradually recover in about one month,which may be one of the reasons of asthenopia postoperatively.
