男性乳腺癌病理分子亚型与预后的相关性分析

摘　　要：目的:研究男性乳腺癌分子亚型与预后的关系。方法:回顾性分析男性乳腺癌患者的临床随访和病理资料,应用χ2检验分析分子亚型与临床病理特征的关系,用Kaplan-Meier生存曲线(Log-Rank法)和Cox风险比例回归模型比较不同分子亚型组间预后的差异。结果:68例男性乳腺癌患者具有完整的病理及随访资料并纳入分析。中位随访时间115个月。Lu-minal A型与非Luminal A型在淋巴结转移状态和脉管瘤栓情况差异有统计学意义,P<0.05;两组复发或转移事件发生情况差异有统计学意义(P<0.05),10年总生存率分别为89.2%和73.2%,差异有统计学意义,P<0.05;而两组在TNM分期、肿瘤大小、组织学分级差异无统计学意义,P>0.05。Cox风险比例回归模型分析验证了分子亚型是重要的预后因素,与预后显著相关,P<0.05。结论:男性乳腺癌分子亚型与某些病理学特征相关,Luminal A型与非Lu-minal A型相比预后更好,分子亚型可能具有预测男性乳腺癌预后和指导治疗的价值。OBJECTIVE： To evaluate the interaction between molecular subtypes of male breast cancer and its prognosis.METHODS： Male breast cancer（MBC） clinical and pathological data were collected;molecular subtype was determined by immunohistochemistry.Overall survival among different subtypes was by Kaplan-Meier method and Cox proportional hazard regression model;Pathological characters associated with molecular subtype was evaluated,using the χ2 test.RESULTS： Sixty-eight patients with complete data were included in analysis.Median follow-up time was 115 months.Comparing to non-luminal A,patients with subtype of Luminal A had less lymph node metastasis and less blood vessel invasion（P0.05）,also with lower recurrent rate and better overall survival（10-year survival rate was 89.2% and 73.2% respectively,P0.05）.There was no significant difference in TMN stage,tumor size and histological grade between two groups（P0.05）.Cox proportional hazard regression model indicated that molecular subtype was an important factor on prognosis（P0.05）.CONCLUSIONS： Molecular subtypes interact in certain characters of pathology and are associated significantly with overall survival of MBC.Group of Luminal A have a better prognosis than non-Luminal A group.Molecular subtypes may have prognosis-predict and treatment-guide value.
