急性重症胰腺炎胃肠功能衰竭防治体会

摘　　要：目的探讨急性重症胰腺炎(serious acute pancreatitis,SAP)采用非手术治疗防治胃肠功能衰竭效果。方法选取我院收治的68例SAP患者,随机分为2组,治疗组50例在应用非手术营养支持综合治疗基础上,加用中药丹参、生大黄等,观察组18例单纯应用禁食、补液、抗感染、抑制胰腺外分泌等常规基础治疗。比较2组治愈率、并发症、死亡等效果。结果治疗组治愈率92.00%、并发胰腺囊肿5.88%、改为手术治疗1例、未发生胃肠功能衰竭者、无一例死亡,观察组治愈率72.22%、并发胰腺囊肿8.82%、并发胃肠功能衰竭8.82%、改为手术治疗1例,死亡1例。2组均取得了良好的治疗效果,并发症的发生率存在着显著差异(P<0.05)。结论非手术综合治疗合并胃肠功能衰竭的SAP有效,中药对防止SAP并发症的发生有着显著效果,尤其对防止胃肠功能衰竭效果明显,值得临床推广,胃肠功能的恢复对阻止重症急性胰腺炎的发展至关重要。Objective To investigate severe acute pancreatitis（serious acute pancreatitis,SAP）using non operation treatment for prevention of gastrointestinal function failure effect. Methods In our hospital,68 patients with SAP,were randomly divided into two groups,50 cases in the treatment group in the application of operation nutritional support on the basis of comprehensive treatment, combined with Danshen,rhubarb,the observation group 18 cases simple application of fasting,rehydration,resistance to infection, inhibition of pancreatic exocrine secretion and other conventional foundation treatment. Comparison of two group cure rate, complications,death and other effects. Results In the treatment group,the cure rate of 92.00%,complicated with pancreatic cyst 5. 88%,changed to operation in treatment of 1 patients with gastrointestinal function failure,has not occurred,no l patients died, the observation group cure rate of 72.22%,8.82%,complicated with pancreatic cyst complicated with gastrointestinal function failure in 8.82%,changed to operation in treatment of I cases,1 cases of death.The two groups were achieved a good therapeutic effect, complication rates have significant difference（P〈~0.05）.Conclusion Discussion of non operation treatment combined with gastrointes- tinal function failure of traditional Chinese medicine on SAP SAP effectively,prevent complications have a significant effect,especially to prevent gastrointestinal function failure effect,worthy of clinical application,the recovery of gastrointestinal unction to prevent the development of severe acute pancreatitis.
