白内障术后盲与低视力的临床分析

摘　　要：目的:探讨白内障小切口囊外摘除加人工晶体植入术后盲与低视力的原因,进一步提高手术疗效及降低医疗纠纷的风险。方法:根据中国盲与低视力诊断标准对29例29眼白内障患者,术后3月最佳矫正视力仍低于0.3者,进行盲与低视力原因分析。结果:白内障术后盲2眼,二级低视力6眼,一级低视力21眼。导致盲与低视力的主要原因为术前已存在除白内障外影响视力恢复的眼部疾患,其中术前可预期的22眼,术后发现的4眼;与手术并发症有关的3眼。结论:白内障患者术后视力的恢复,与患者自身的眼部情况有直接关系,详细的术前检查评估,精细的手术操作及良好的医患沟通对降低术后医疗纠纷至关重要。Objective： To explore the cause of blindness and low vision after small incision extracapsular cataract extraction and intraocular lens（IOL） implantation in order to improve operation efficacy and reduce the risk of medical disputes.Methods： According to China＇s diagnostic criteria of blindness and low vision,the cause of blindness and low vision in 29 cataract patients（29 eyes） was analyzed,whose best corrected visual acuity is still less than 0.3,3 months after operation.Results： After cataract surgery,there was blindness in 2 eyes,low vision with Level 2 in 6 eyes,and low vision with Level 1 in 21 eyes.The main cause of blindness and low vision is the existence of other vision-affecting eye diseases before operation in addition to cataracts,which could be preoperatively predicted in 22 eyes,postoperatively found in 4 eyes,related to surgical complications in 3 eyes.Conclusions： The recovery of visual acuity in cataract patients has a direct relation to the patients＇ own eye condition.Detailed preoperative assessment,carefully-conducted operation and effective communication between doctors and patients is of great importance to reducing medical disputes.
