彩超筛查法对产前胎儿颅脑畸形诊断价值

摘　　要：目的：探讨彩超筛查法对产前胎儿颅脑畸形的诊断价值，为临床处置提供决策信息。方法：对13760例妊娠18—40周孕妇常规超声产前检查，对其中145例疑诊有胎儿畸形者采用彩超筛查。回顾性分析确诊为胎儿颅脑畸形病例的声像图特征。结果：13760例胎儿中，产前采用彩超筛查法诊断颅脑畸形145例，均经引产或出生后证实，彩超筛查法诊断符合率为100％（145／145）。其中87例合并其他系统畸形，产前诊断79例，漏诊8例，漏诊率5．52％。结论：彩超筛查法对产前胎儿颅脑畸形的诊断具有重要的临床价值。但合并其他系统畸形检出的准确性尚需进一步提高。Objective：To evaluate color Doppler US prenatal diagnostic value for fetal skull brain de- formities in order to make clinical effective remedies. Methods： 13 760 pregnant women with gestational age of 18 -40 weeks were prenatally examined by routine obstetric ultrasound. Among them, 145 cases suspected fetal anomaly were further examined by color Doppler US to make definite diagnosis. The fetal skull brain deformity images were analyzed. Results：In 13760 fetal cases, 145 cases with fetal skull brain deformities were confirmed by post - natal evaluation or abortion . 145 cases were verified on color Doppler US. Color Doppler US prenatal diagnostic coincidence rate of fetal skull brain deformities was 100% （145/145） . Among 87 cases complicated the other system anomalies ,79 cases were prenatally diagnosed ,8 cases were missed diagnosis ,the rate of inconsistency was 5.52%. Conclusions ： Color Doppler US provides important valuable information for diagnosis of fe- tal skull brain deformities, but suspected skull brain deformity cases and the diagnostic accuracy of complicating the other system anomalies need further improvement.
