摘　　要：目的 探讨低浓度平阳霉素注射治疗龟头部静脉畸形的疗效.方法 对29例龟头部静脉畸形患者采用低浓度平阳霉素（0.5mg/ml）经皮穿刺注射治疗病变区域.每次治疗注射药液量为1～3ml,并在阴茎体部病变区近心端置止血带,自注射时到注射后5min阻断龟头部的血液回流,以保持静脉畸形体内的药物浓度及含量.治疗后,外用抗生素软膏,纱布适度加压包扎.根据病变情况决定重复治疗的次数,每次治疗间隔15d.结果 16例患者治疗3次,8例患者治疗4次,5例患者治疗6次.对所有患者随访12～38个月.26例患者治愈,3例患者有效,均无复发倾向.阴茎勃起和排尿功能正常,外观满意.结论 低浓度平阳霉素注射治疗安全有效,是龟头部静脉畸形的首选治疗方法.Objective To explore the clinical effect of venous malformations of the glans penis treated with low concentration Pingyangmycin injection. Methods Totally 29 patients with venous malformation of the glans penis were treated with low concentration Pingyangmyein injection, in which concentration was 0.5 mg/ml, through percutaneous injection, and the volume of the solution varied from 1 to 3 ml per injection every time. Before treatment, a tourniquet was placed around the penile shaft, close to the heart. The tourniquet remained in position during the injection and 5 minutes after the injection, to contain Pingyangmycin within the venous malformation. After treatment, antibiotic cream was applied over the lesion and compression applied with gauze. Treatment was performed in several stages. The applications were carried out after intervals of 15 days. Results Sixteen patients received 3 therapies, 8 cases received 4 therapies, and 5 cases received 6 therapies. After 12-38 months followed-up in all cases, 26 patients were cured and 3 patiens had marked improvement. No patients presented with signs of recurrence. All patients had normal penile sensation and erections and satisfied with the appearance. Conclusion Injection of low concentration Pingyangmycin is proved to be a safe, effec- tive therapy and should be considered first among the available therapeutic modalities for venous malformations of the glans penis.
