腹腔镜下巨大子宫肌瘤切除术中出血量增多因素探讨

摘　　要：目的探讨腹腔镜下巨大子宫肌瘤切除术中出血量增多的因素。方法将76例巨大子宫肌瘤患者分为超大肌瘤组(A组)、多发肌瘤组(B组)、伴子宫腺肌病组(C组)以及单纯巨大肌瘤组(D组)4组,观察并对比4组手术时间、术中出血量、住院时间及并发症。结果 A、B、C 3组手术时间、术中出血量明显大于D组(P<0.05),A、B、C 3组之间手术时间、术中出血量无显著差异,4组住院时间及并发症无显著差异。结论腹腔镜下巨大子宫肌瘤切除术中出血量增多与肌瘤的体积、数目,以及是否伴有子宫腺肌病具有密切关系,术前对这些情况进行评估并采取相应措施,对减少术中出血具有重要意义。Objective To explore the factors of increased bleeding amount during laparoscopic huge hysteromyomectomy.Methods Seventy-six patients with huge uterine fibroids were divided into extra large fibroids group（group A）,multiple fibroids group（group B）,adenomyosis group（group C） and simple huge fibroids group（group D）.The operation time,bleeding amount,length of stay and complications among the four groups were compared.Results The operation time and bleeding amount in groups A,B,C were obviously higher than those in group D（P〈0.05）.The operation time and bleeding amount among groups A,B and C showed no obvious difference.The length of stay and complications among the four groups had no obvious difference.Conclusion The increased bleeding amount during laparoscopic huge hysteromyomectomy is closely related to the volume,number of fibroids and accompanied adenomyosis.It is important to assess these conditions before surgery in order to reduce bleeding in operation.
