两种持续性操作测试诊断注意缺陷多动障碍的临床应用

摘　　要：目的:探讨视听持续性操作测试(TOVA)和整合视听连续执行测试(IVACPT)在诊断儿童注意缺陷多动障碍(ADHD)中的应用价值。方法:对门诊疑似ADHD的267例儿童采用精神障碍诊断与统计(DSM-IV)标准进行诊断,分别对152例进行TOVA,115例进行IVACPT,比较两组检测结果并与DSM-IV标准比较。结果:TOVA和IVACPT对ADHD的阳性检出率分别是81.08%和95.24%,都与DSM-IV诊断结果具有相关性,TOVA检测与DSM-IV诊断差异有统计学意义,IVACPT检测与DSM-IV诊断差异无统计学意义。IVA的敏感度显著高于TOVA。结论:IVACPT和TOVA对ADHD的诊断都具有客观的应用价值,IVACPT优于TOVA,对临床诊断ADHD有极高价值。Objective： To explore the application values of continuous performance test on visual and auditory （TOVA） and integrated visual and auditory continuous performance test （IVACPT） in diagnosis of attention deficit hyperactivity disorder （ADHD） among children. Methods： A total of 267 children suspected of ADHD from outpatient department were diagnosed according to diagnostic and statistical manual of mental disordersIV （DSM IV） , 152 children underwent TOVA and 115 children underwent IVACPT, the results were compared between the two groups, and the results were compared with DSMIV. Results： The positive detection rates of TOVA and IVACPT for ADHD were 81.08% and 95.24%, respectively. The results of TOVA and IVACPT were correlated with the diagnostic results of DSMIV, respectively, and there was significant： difference between TOVA and IVACPT, there Was no significant difference between IVACPT and DSMIV. The sensitivity of IVACPT was significantly higher than that of TOVA. Conclusion： Both IVACPT and TOVA have objective application values for diagnosis of ADHD, IVACPT is superior to TOVA, and it has very high value for clinical diagnosis of ADHD.
