摘 要:目的 探讨MRI对椎管内海绵状血管瘤的诊断及其鉴别诊断价值.方法 9例经手术病理证实的椎管内海绵状血管瘤行磁共振平扫检查,同时行增强扫描,分析其影像表现.结果 5例位于胸段、2例位于颈段、2例位于腰骶段.海绵状血管瘤的MRI表现:6例呈等T1WI信号,2例呈长T1WI信号,1例呈短T1WI信号,9例呈长T2WI信号影,增强扫描4例明显均匀强化,2例不均匀强化,3例无明显强化.MRI信号因海绵状血管瘤反复出血不同时期出血成分沉积及血栓形成、胶质增生而表现多样.结论 MRI对椎管内海绵状血管瘤的术前确诊及鉴别诊断具有重要价值.Objective To explore the value of MRI in the diagnosis and differential diagnosis of cavernous hemangioma of spinal canal. Methods 10 cases with histologically proved cavernous hemangioma of spinal canal underwent plain MR imaging,of them,9 cases underwent contrast-enhanced MR imaging, simuhaueously. Results The lesions located at thoracic spine in 5 cases, cervical spine in 2 cases and lumbar spine in 2 cases. MRI features of cavernous hemangiomas:the lesions showed iso-intensity in 6,hypo-in- tensity in 2 and hyper-intensity in 1 on T1 WI, hyper-intensity in 9 on T2 WI. On contrast-enhanced images, the lesions enhanced homogeneously in 4,enhanced heterogeneously in 2 and no enhancement in 3. MRI appearances of cavernous hemangiomas were variant clue to pathologic changes of cavernous hemangioma in different period such as bleeding,deposit of hemosiderin and colloid hyperplasia. Conclusion MRI is the most specific and sensitive method in diagnosis and differential diagnosis of cavernous hemangioma in spinal canal.
