摘 要:目的检测骨形成蛋白-2(bone morphogenetic protein-2,BMP-2)和p21^(WAF1)在乳腺癌患者原发灶及骨转移灶中的表达,探讨它们在乳腺癌骨转移发生中的意义。方法选择临床及病理资料完整的乳腺癌患者126例,其中未发生骨转移者88例,发生骨转移者38例。应用免疫组织化学方法检测原发灶及骨转移灶中BMP-2和p21^(WAF1)的表达情况,并结合患者年龄、肿瘤大小、淋巴结转移、临床分期等,探讨它们之间的相互关系。结果①乳腺癌组织中BMP-2和p21^(WAF1)的阳性表达率在有骨转移、有淋巴结转移组中均低于无骨转移、无淋巴结转移组(P<0.01)。②临床Ⅲ~Ⅳ期的乳腺癌组织BMP-2和p21^(WAF1)的阳性表达率分别为20.59%、23.53%,明显低于其在临床Ⅱ期乳腺癌组织中的表达(P<0.01)。③BMP-2阳性表达率在骨转移灶中的表达明显低于其在原发灶中的表达(P<0.05),p21^(WAF1)阳性表达率在骨转移灶中也低于原发灶,但差异无统计学意义(P>0.05)。④BMP-2和p21^(WAF1)蛋白阳性表达率呈正相关(r=0.887,P<0.01)。结论①乳腺癌合并淋巴结转移时骨转移的发生率高。②临床分期越晚,骨转移的发生率越高。③BMP-2和p21^(WAF1)高表达着淋巴结转移少,发生骨转移的机会较少;而BMP-2和p21^(WAF1)的低表达预示具有形成骨转移的潜能。Objective To investigate the expression and significance of 1021wan and bone morphogenetic protein -2 (BMP -2) with bone metastasis in patients with breast cancer. Methods A total of 126 breast cancer patients including 38 cases with bone metastasis were analysed. The expressions of BMP -2 and p21wAF1 were detected by immunohistochemical staining. The correlations among the expressions of BMP - 2, p21WAF1 and clinical stage, positive lymph node and tumor size were analysed. Results (1)The positive expression of BMP - 2 and p21wan in the group of breast cancer with bone metastasis was lower than that in the group without bone metastasis ( P 〈 0.01 ). The same result was obtained between the group of breast cancer with axillary lymph node metastasis and the group of no lymph node metastasis. (2)The positive expression of BMP - 2 and p21 WAF1 in stages Ⅲ - Were 20.59% and 23.53% respectively and siginificantly lower than those of stage Ⅱ ( P 〈 0.01 ). (3)The positive expression of BMP-2 in tissue of bone metastasis was lower than that of breast cancer(P 〈 0.05 ). Thepositive expression of p2WAF1 in tissue of bone metastasis was lower, too, but there was no significant difference ( P 〉 0. 05 ). 4There was a positive correlation between the expression of BMP - 2 and p21 WAF1 ( r = 0. 887,P 〈 0. 01 ). Conclusion Patients with high expression of BMP -2 and p21 WAF1 have less risk of lymph node metastasis and bone metastasis. The low expression of BMP - 2 and p21 WAF1 may indicate the potential of bone metastasis.
