腹腔镜辅助下直肠外翻拖出式手术在中低位直肠癌治疗中的无瘤技术与临床价值

摘　　要：目的探讨腹腔镜辅助下直肠外翻拖出式手术在中低位直肠癌治疗中的无瘤技术与临床价值。方法将中低位直肠癌患者46例随机分成观察组24例和对照组22例,观察组采用腹腔镜辅助下直肠外翻拖出式手术,对照组采用直肠癌低位前切除术,比较分析2组手术时间、术中出血量、肿瘤指标及术后恢复情况。结果观察组手术时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);观察组肿块下缘距远切端距离长于对照组,清扫淋巴结的总数多于对照组,差异有统计学意义(P<0.05);且观察组患者肠道功能恢复时间明显优于对照组(P<0.05)。2组并发症发生情况比较,差异无统计学意义(P>0.05)。结论腹腔镜辅助下直肠外翻拖出式手术中低位直肠癌的治疗可以达到无瘤原则,临床效果好,值得推广应用。Objective To investigate the non- tumor technique and clinical value of laparoscope - assisted transanal excision in the treatment of mid - low rectal cancer. Methods Forty - six patients with mid - low rectal cancer were randomly divided into observation group of 24 cases and control group of 25 cases. The observation group was treated with laparoscope- assisted transanal excision,the control group was treated with low anterior resection of rectal cancer. The operation time, blood loss, tumor markers and postoperative recovery of two groups were compared and analyzed. Results In the observation group, the operation time was longer than the control group, blood loss was less than the control group, there were significant differences between two groups （ P 〈 0.05 ）. In the observation group,the distance from the inferior margin of the tumor to the distal incisal edge was longer than the control group, the total number of harvested lymph nodes was more than the control group, there were significant differences between two groups （P 〈 0.05 ）. And the intestinal function recovery time of the observation group was significantly better than the control group （P 〈 0.05）, but the complications of two groups were similar without significant difference （P 〉 0.05 ）. Conclusion Non - tumor principles can be achieved by laparoscope - assisted transanal excision in the treatment of mid - low rectal cancer. It has good clinic effect, and deserve to be widely applied.
