尼莫地平联合多奈哌齐对脑卒中后认知功能障碍的影响

摘　　要：目的:评价尼莫地平联合多奈哌齐治疗脑卒中后认知功能障碍的疗效。方法:将符合纳入标准的患者随机分为尼莫地平+多奈哌齐组(试验组)、尼莫地平组(对照组1)和多奈哌齐组(对照组2),三组患者均同时给予脱水、抗凝、改善脑供血等基础治疗,利用MMSE、HDS及ADL(改良Barhtel指数)量表评定疗效。结果:三组患者治疗后的MMSE、HDS及Barhtel得分较治疗前均有不同程度增加,差异均有统计学意义;试验组和对照组2治疗后12周与治疗后8周相比,MMSE、HDS及Barhtel得分均显著增加,差异有统计学意义;试验组治疗后8周和12周的MMSE、HDS及Barhtel得分均高于同期对照组,差异有统计学意义,对照组2治疗后8周和12周的MMSE、HDS及Barhtel得分均高于同期对照组1,差异有统计学意义。结论:尼莫地平联合多奈哌齐能有效改善脑卒中后的认知功能障碍和日常生活活动能力。Objective：Evaluation of the efficacy of nimodipine combined with donepezil on the cognitive dysfunction after stroke patients. Methods： Patients who meet the inclusion criteria were randomly divided into nimodipine ＋ donepezil（test group）, nimodipine group （control groupl） and donepezil group（control group 2） in which at the same time given patients of the three groups basic treatment were dehydration,anticoagulation and improving cerebral circulation. Curative effect in different groups were evaluated by MMSE, HDS and the ADL（modified Barhtel index）. Results： MMSE, HDS and Barhtel score increased in varying degrees of the three groups of patients after treatment in which the difference was statistically significant. MMSE, HDS and Barhtel score of test group and control group 2 in treatment after 12 weeks compared to treatment after 8 weeks were all increase in which the difference was statistically significant. After 8 weeks and 12 weeks treatment, the test group score of MMSE, HDS and Barhtel were higher than that in the same period of the control group in which the difference was statistically significant. The MMSE, HDS and Barhtel score in treatment ofter 12 weeks of the control group were higher than that of the corresponding control group in which the difference was statistically significant. Conclusion： Nimodipine combined with donepezil can effectively improve the cognitive dysfunction and activities of daily living ability after stroke patients.
