全身化疗联合局部热疗治疗晚期胃癌的临床观察

摘　　要：[目的]观察全身化疗联合局部热疗治疗晚期胃癌的近期疗效及不良反应。[方法]将2007年10月至2010年8月收治的晚期胃癌患者113例,分为热化疗联合治疗组和单纯化疗组。观察患者的近期疗效、临床受益率、KPS评分、疼痛分级以及不良反应。[结果]近期总有效率(CR+PR)热化疗组35.3%,单纯化疗组30.6%,两组差异没有统计学意义(χ2=0.298,P=0.585);临床获益率(CR+PR+SD)热化疗组86.3%,单纯化疗组43.5%,两组差异有统计学意义(χ2=6.136,P=0.047)。热化疗组患者生活质量改善,疼痛缓解明显,不良反应主要包括胃肠道、肝脏毒性及骨髓抑制。[结论]全身化疗联合局部热疗治疗晚期胃癌疗效肯定,临床获益率明显提高,耐受性好,值得临床推广。[Purpose] To investigate the efficacy and side effects of systemic chemotherapy combined with local hyperthermia in patients with advanced gastric cancer.[Methods] One hundred and thirteen patients with advanced gastric cancer were divided into two groups：group HC received chemotherapy and regional hyperthermia;group C received chemotherapy alone.[Results] The overall response rates were 35.3% and 30.6% in group HC and group C respectively,with no significant difference（χ2=0.298,P=0.585）.The clinical benefit response rate in group HC was significantly higher than that in group C（86.3% vs 43.5%,χ2=6.136,P=0.047）.The quality of life improved and the pain relieved markedly in group HC.The main toxicities were gastrointestinal tract,liver toxicity,and myelosuppression.[Conclusion] Systemic chemotherapy combined with local hyperthermia might be an effective and safe treatment for patients with advanced gastric cancer.
