认知行为疗法对首发女性偏执型精神分裂症康复期焦虑情绪的疗效观察

摘　　要：目的:观察认知行为疗法对首发偏执型精神分裂症康复期女性患者焦虑情绪的疗效。方法:选取首发偏执型精神分裂症康复期患者100例,随机分为研究组与对照组各50例。研究组在抗精神病药物治疗的基础上给予连续28天、每天1个小时的认知行为疗法;对照组只给予原有药物治疗。在研究结束后进行心理治疗,以符合伦理学原则。在治疗前、治疗后以及治疗后12周时进行HAMA量表评分,观察焦虑疗效。结果:研究组脱落15例,对照组脱落10例。研究组35例患者治疗前后HAMA评分分别为(11.03±1.30)分、(4.30±1.84)分,治疗后评分显著低于治疗前,差异具有统计学意义(t=5.083,P=0.005)。治疗后研究组HAMA评分(4.30±1.84)分,对照组(5.02±2.33)分,研究组低于对照组,差异具有统计学意义(t=-4.395,P=0.000)。12周随访研究组HAMA评分(3.58±1.47)分,仍显著低于治疗前,差异均具有统计学意义(t=4.120,P=0.043)。结论:认知行为治疗有助于改善康复期偏执型精神分症女性患者的焦虑情绪,在12周随访时疗效依然存在。Objective： To observe the curative effect of cognitive behavioral therapy on the convalescent female paranoid schizophrenia anxiety. Methods ：100 convalescent patients with paranoid schizophrenia were randomly divided into study group （50 cases） and control group （50 cases）, based on antipsychotic treatment, study group used cognitive behavioral therapy, the control group only used original drug treatment. Psychological treatment was used at the end of the study. HAMA scale score before treatment, after treatment and 12 weeks after treatment were used to observe the change of anxiety. Results：Study group come off 15 cases,and control group come off 10 cases. In study group, HAMA scale score after treatment[ （4.30 ± 1.84） points] were significantly lower than before treatment[ （11.03 ± 1.30 ）points] ,there were statistical differences（ t = 5. 083 ,P = 0. 005 ）. After treatment, HAMA scale score of study group [ （4.30 ± 1.84 ） points ] were lower than control group [ （ 5.02 ± 2.33 ） points ], there were statistical differences （ t = - 4.395, P =0.000）. In study group, HAMA scale score after 12 weeks of treatment[ （ 3.58 ± 1.47 ） points] were significantly lower than before treatment, there were statistical differences（ t = 4.120, P = 0. 043 ）. Conclusion：Cognitive behavioral therapy can improve anxiety of convalescent female paranoid schizophrenia.
