基层医院开展门诊白内障防盲手术的临床观察

摘　　要：目的总结在基层医院门诊开展白内障防盲手术手术的临床效果。方法 356例(359眼)白内障均按门诊手术程序进行准备,排除眼部及全身绝对手术禁忌证,针对相对手术禁忌证进行先期治疗,手术方法按晶状体核的硬度采用超声乳化术(II～Ⅲ级核)或现代囊外摘除术(IV～V级核),并对比两者在术后第1天裸眼脱盲率、脱残率及术后并发症方面的差异。结果全部病例术后第1天总的裸眼脱盲率为89.67%,裸眼脱残率为47.07%,但超声乳化组和现代囊外摘除组裸眼脱残率存在差异。结论基层医院在门诊开展白内障防盲手术安全、有效,能取得较好的术后脱盲率,但超声乳化术在术后脱残率上较囊外摘除术高,且在术后并发症方面角膜水肿更少,提示在门诊条件下开展白内障超声乳化的防盲手术更安全,同时今后应注意加强对防盲手术术后的随访。Objective To summarize the effects of cataract blindness surgery operations in primary hospital.Methods 356 cases（359 eyes） are all prepared by outpatient surgical procedures,any ocular and systemic contraindications surgery are absolutely ruled out.Results contraindications to surgery for early treatment,surgical methods used by the hardness of the lens nucleus phacoemulsification（II～III class nuclear） or modern extracapsular cataract extraction surgery（IV～V grade nuclear）,and compare the two in the first postoperative day naked eye literacy rate,off rate and the residual differences in postoperative complications.Results All patients after the first day of the general literacy rate of 89.67% the naked eye,the naked eye off the residue was 47.07%,but the phacoemulsification group and the modern extracapsular cataract extraction group naked eye off the disability rate is different.Conclusion Outpatient cataract blindness surgery carried out in the primary hospital is safe,effective,it can achieve good literacy rate.But phacoemulsification in patients off disability rate than extracapsular cataract extraction surgery on high,and in fewer postoperative complications corneal edema.This indicates： under outpatient conditions,cataract surgery is safer.To prevent blindness,and blindness in the future,close attention should be paid to postoperative follow-up.
