白质脑病的临床及MRI影像特点分析

摘　　要：目的探讨白质脑病的病因、临床表现以及MRI影像学特点。方法回顾分析我院2009年12月～2010年3月53例白质脑病患者的临床及影像资料。结果本组白质脑病病因为血管性白质脑病31例,多发性硬化8例,可逆性后部白质脑病6例,一氧化碳中毒性脑病4例,艾滋病痴呆综合征、肾上腺脑白质营养不良各1例,未能明确病因2例。临床表现以认知障碍、偏瘫及抑郁等精神异常为主;MRI表现为双侧脑室周边及半卵圆中心对称的片状或融合的长T1、长T2信号。结论白质脑病病因复杂,患者均有不同程度的认知障碍,MRI对诊断脑白质病敏感性高,有重要的临床参考价值。Objective To study the features of leukoencephalopathy＇s etiology,clinical manifestations and MR imaging.Methods The clinical and imaging data of 53 patients with leukoencephalopathy were retrospectively analyzed.Results Vascular leukoencephalopathy accounted for the highest proportion of leukoencephalopathy（n=31）.8 patients suffered from multiple sclerosis,6 patients from reversible posterior leukoencephalopathy,4 patients from in carbon monoxide poisoning,1 patient from AIDS dementia syndrome,1 patient from adrenoleukodystrophy,2 patients failed to confirm the cause.Clinical manifestations were mainly cognitive impairment,paralysis,depression and other mental disorders;MRI showed long T1 and short T2 signals in periphery of bilateral ventricle and the center of semi-oval＇s sheet or fusion.Conclusion The causes of leukoencephalopathy are complex,and the patients have common cognitive impairment.MR imaging has high sensitivity and important clinical values in the diagnosis of white matter diseases.
