摘  要:目的比较自锁托槽和传统托槽不拔牙矫治对于软硬组织的变化及牙弓形态的变化。方法选择36例安氏Ⅰ类患者分为自锁托槽组和传统托槽组,分别为18例。分别使用自锁托槽和传统托槽进行不拔牙矫治,对治疗前后的头颅侧位片及模型进行测量,并对测量结果进行配对和成组t检验。结果矫治后2组下牙弓周长都有增长,自锁组下牙弓周长增加3.29 mm,传统组增加1.67 mm,差异有统计学意义(P<0.05);自锁托槽组和传统托槽组牙弓各段宽度都有一定程度的扩大,但是2种托槽之间差异没有统计学意义;自锁托槽组LL-EP增加0.63 mm,传统托槽组LL-EP增加1 mm,差异有统计学意义(P<0.05);∠U1-NA、∠L1-NB自锁组较普通组少增加1°,但是2组之间没有统计学差异(P<0.05);2组的SNA、SNB、ANB、MP/SN角的治疗前后变化量差异无统计学意义。结论应用自锁托槽和传统托槽不拔牙矫治牙列拥挤时,2种托槽都不能改变骨骼关系,都有一定程度的扩弓作用和使上下前牙唇倾,但两者之间的差异没有统计学意义。Objective To evaluate changes in the dental arch and soft and hard tissue produced by the self-ligating bracket and conventional bracket with non-extraction treatment.Methods Thirty six patients diagnosed as class Ⅰ were selected in the study.The patients were assigned to 2 groups:group 1 received non-extraction treatment with a self-ligating bracket and the other group with a conventional bracket.Lateral cephalometric radiographs and dental casts obtained at the beginning and the end of treatment were used to assess the alteration in soft and hard tissue,and measurements of intercanine and intermolar and interpremolar width and arch length of dentition were made on dental casts to investigate changes associated with the correction.Paired t-tests and group t-tests were used in statistical analysis.Results There was significant increase in length of dental arch with self-ligating and conventional bracket after treatment,self-ligating group increasing 3.29 mm and conventional group increasing 1.67 mm,there being significant differences between self-ligatiing and conventional group;both group were enlarged in the dental widths and there was no significant difference in dental width between two groups;also there was significant increase in LL-EP in self-ligating bracket group,but not in conventional bracket group;although self-ligating group increased less by 10 in ∠U1-NA、∠L1-NB than conventional group,there were no significant difference between self-ligating and conventional bracket groups;there was no significant change in SNA、SNB、ANB、MP/SN between self-ligating and conventional bracket groups.Conclusions At the present stage,we apply self-ligating and conventional bracket to treat dentition crowing with non-extraction,and both of them can not change the relationship of sekeleten;self-ligating bracket and conventional bracket can cause the proclination of the incisors and increase the arch width and length to some degree,but no significant difference is found between self-ligating bracket and conve
