除皱切口及颞浅筋膜瓣在腮腺全切术中的应用

摘　　要：目的:探讨应用除皱切口联合颞浅筋膜瓣在腮腺全切除术中应用的意义。方法:回顾92例腮腺全切除术。对照组30例采用常规S型切口,未行修复;实验组62例采用除皱切口联合颞浅筋膜瓣覆盖腮腺床,术后复诊了解是否存在味觉-出汗综合征。术区面部外形分级评定。结果:术后平均随访期18个月。术后味觉-出汗综合征对照组8例(27.0%),实验组4例(6.5%)。面部外形评级对照组Ⅲ级22例,Ⅳ级8例;实验组Ⅰ级16例,Ⅱ级46例。两组差异有统计学意义。结论:应用除皱切口联合颞浅筋膜瓣在腮腺全叶切除术后对面部外形的恢复及味觉-出汗综合征的预防有较好效果。Objective： To evaluate the significance of face lift incision combined with temporal fascial flap in total parotidectomy.Methods： The medical records of 92 cases underwent total parotidectomy were reviewed.Blair incision without reconstruction of parotid bed was conducted in 30 cases（control group）.Face lift incision combined with temporal fascial flap for reconstruction of parotid bed was used in 62 cases（test group）.Postoperatively,the presence or absence of Frey syndrome was identified.The aesthetic results were evaluated.Results： The mean follow-up was 18 months.The presence of Frey syndrome was identified in 8 cases（27.0%） in control group and 4 cases（6.5%） in test group.Aesthetically,22 cases were graded as Ⅲ and the rest 8 cases were Ⅳ in control group;however,16 and 46 cases received an evaluation of Ⅰand Ⅱ in test group（P0.05）.Conclusion： Face-lift incision combined with temporal flap is effective in improving aesthetical results and preventing Frey syndrome in total parotidectomy.
