老年期抑郁症患者米氮平治疗前后探究性眼动分析的研究

摘　　要：目的:探讨老年期抑郁症患者探究性眼动(Exploratory Eye Movement,EEM)的特点及其对疗效评估的意义。方法:纳入60岁以后起病的符合美国精神障碍诊断与统计手册第4版(Diagnostic and Statistic Manual of Mental Disorders 4th,DSM-Ⅳ)的抑郁症患者32例和正常对照32名,实验组给予米氮平治疗6周,实验组治疗前后和对照组均进行EEM测定。结果:实验组治疗前较治疗后及正常对照组凝视点数(NEF)和反应性探索评分(RSS)均明显降低(P<0.05),而实验组治疗后与正常对照组相比较NEF和RSS均无明显差异(P>0.05);以治疗6周后HAMD总分减分率≥50%为有效进行分组,有效组(n=19)较无效组(n=13)RSS明显增多(P<0.05)。结论:老年期抑郁症患者存在EEM异常,EEM可作为反映老年期抑郁症患者病情和疗效的一项生物学指标,米氮平是一种理想的抗抑郁药物,特别是对老年期抑郁症患者的认知功能的恢复有较好地改善作用,值得临床推广使用。Objective：To explore the characteristic of the exploratory eye movement （EEM） on depression Patients in the eld- erly, and explore the significance of evaluate therapeutic efficacy. Methods：There are depression patients 32 cases who onset after the age of 60 and consistent with Diagnostic and Statistic Manual of Mental Disorders 4th （ DSM - IV ） and 32 cases normal control, the pa- tients were treated six weeks with mirtazapine, the people in the experimental group before and after treatment and control groups test EEM. Results： The number of eye fixation （NEF） and the responsive search score （RSS） in the experimental group before treatment were significant lower than those in the experimental group after treatment and the normal control group （ P 〈 0.05 ） ,while the comparison between experimental group after treatment and the control group on NEF and RSS, they have not significant difference （ P 〉 0. 05）. The patients were divided into effective group 19 cases and ineffective group 13 cases after treated six weeks, as according to HAMD reduces the fraction ≥50%, the number of RSS in the effective group were significant more than those in the ineffective group （ P 〈 0.05 ）. Conclusions ： The EEM of depression Patients in the elderly are exceptional, EEM can become the objective biology tar- get of reflection the old age depression patientg condition and the treatment result. The mirtazapine is a kind of ideal antidepressed medicine, it has the good function to the old age depression patientg function improvement, and it is worth to be promoted in the clinical.
