经肛门PPH偏心性切除吻合治疗直肠良性肿瘤

摘　　要：目的探讨经肛门PPH偏心性切除吻合在直肠良性肿瘤治疗中的应用价值。方法选择40例距肛门10 cm内,直径>1 cm的的直肠良性肿瘤患者,经肛门PPH偏心性切除吻合作为治疗组;同期20例传统经肛门手术切除作为对照组,对比观察两组术后并发症发生率、肛门功能评定、术后住院时间。结果治疗组并发症发生率(20%)低于对照组(45%);术后1、3个月肛门功能评定优良率,治疗组明显高于对照组(P=0.022、0.023<0.05);治疗组住院时间为(4.1±1.5)d,对照组为(4.5±1.6)d,两组间比较差异无统计学意义(P>0.05)。结论经肛门PPH偏心性切除吻合治疗直肠良性肿瘤是一种安全、有效、微创的治疗方法,值得应用和推广。Objective To explore the value of procedure for prolapse and hemorrhoids（PPH） through the anus to the eccentric resection and anastomosis in the treatment of benign tumors of rectum.Methods 40 Patients with benign tumors of the rectum which were within 10 cm from the anus and diameter greater than 1cm,were selected and underwent PPH through the anus eccentric resection and anastomosis,as the treatment group.20 cases of conventional surgery through the anus were served as a control group in the same period.Compared the incidence of postoperative complications rate,anal functional assessment and postoperative hospital stay in the two groups.Results Complications rate in the treatment group（20%） was lower than that in the control group（45%）.Excellent rate of anal functional assessment between the two groups after 1 and 3 months were compared,excellent rate of treatment group was significantly higher（P =0.022,0.023 0.05）.Hospital stay of treatment group was（4.1±1.5） days,control group was（4.5±1.6） days,in which there was no significant difference.Conclusion The method of PPH through the anus eccentric benign tumor resection and anastomosis in rectum benign tomors is safe,effective and minimally invasive,which should be applied and popularized.
