牙周牙髓联合治疗120例重度牙周炎的临床分析

摘　　要：目的探讨根管治疗结合局部缓释抗菌治疗重度牙周炎的疗效。方法选取2008年8月~2011年6月我院门诊重度牙周炎患者60例,患牙共84颗,随机分为三组：根管治疗＋派丽奥-布洛芬缓释凝胶治疗组、根管治疗＋碘甘油组及碘甘油组,每组各28颗牙。分别记录比较治疗前、治疗后的第4周、第7周、第12周患者的牙龈指数（GI）、菌斑指数（PLI）、探针深度（PD）、龈沟出血指数（SBI）值。结果①根管治疗＋派丽奥-布洛芬缓释凝胶治疗组与根管治疗＋碘甘油组的PLI、SBI、GI、PD指标均优于碘甘油（P〈0.05）;根管治疗＋派丽奥-布洛芬缓释凝胶治疗组优于根管治疗＋碘甘油组（P〈0.05）。②根管治疗＋派丽奥-布洛芬缓释凝胶治疗组与根管治疗＋碘甘油组总有效率高于碘甘油（P〈0.01）;根管治疗＋派丽奥-布洛芬缓释凝胶治疗组总有效率高于根管治疗＋碘甘油组（P〈0.05）。结论根管治疗结合派丽奥-布洛芬缓释凝胶治疗重度牙周炎疗效显著,值得临床推广应用。Objective To explore the effects of periodontal-endodontic plus periocline-ibuprofen compound treatment in patients with severe periodontitis.Methods 60 cases of patients with severe periodontitis would be admitted to our department,84 teeth were randomly divided into experimental groups（periodontal-endodontic treatment plus Periocline-ibuprofen compound group and periodontal-endodontic treatment plus iodine glycerin group） and control group（simple iodine glycerin treatment group）;the PPD,PLI,SBI value of cases were recorded before treatment,treatment after 4 weeks,7 weeks,12 weeks.Results The treatment course of all patients were finished completely.Compared with before treatment,the change of clinical indicators of after treatment in three groups were no statistically significant（P 0.05）,and the clinical indicators of experimental group were improved more significantly,and the difference between the two experimental groups was significant statistical significance（P 0.01,P 0.05）,compared with the control group,the total effective power of two experimental groups were statistically significant,and between the two experimental groups were also statistically significant.Conclusion Periodontal-endodontic plus Periocline-ibuprofen compound treatment in patients with severe periodontitis has a more obvious effect than others.
