经肛门外翻双吻合器保肛术治疗低位直肠癌的临床研究

摘　　要：目的:评价经肛门外翻双吻合器保肛术治疗低位直肠癌的手术方法及疗效。方法:对经肛门外翻双吻合器保肛术治疗低位直肠癌30例患者的临床资料进行分析。结果:30例低位直肠癌患者均保留肛门。无手术死亡,术后发生切口感染3例,吻合口瘘2例。术后4周能预知和控制排便。术后6个月,每日排便1～4次者25例,5例偶有粪便外溢。随访6～70个月,吻合口狭窄1例,局部复发2例,中位生存期23.5个月,1、3和5年生存率分别为88%、68%和48%。结论:经肛门外翻双吻合器保肛术治疗低位直肠癌具有操作简单、安全等优点,能提高患者生存质量。OBJECTIVE： To evaluate the effect of eversion-through combined with double stapling technique for sphincter preservation operation in low rectal carcinoma.METHODS： Eversion-through combined with double stapling technique for sphincter preservation operation was performed on 30 patients with low rectal carcinoma.The clinical data was analyzed retrospectively.RESULTS： Fecal continence was preserved successfully in all patients.There was no operative death.Two cases got anastomosis leak and 3 cases had incision infection after operation.All patients could sense stool and control defecation in 4 weeks after operation.There were 25 cases with 1~4 defecations every day,5 cases with occasional overflow incontinence after half a year.All patients were followed upfrom 6 to 70 months.One case got anastomotic stenosis,2 cases got local recurrence,the median survival time was 23.5 months.The overall 1-,3-and 5-years survival rates were 88%,68% and 48% respectively.CONCLUSION： Eversion-through combined with double stapling technique for sphincter preservation operation is simple and safe for low rectal carcinoma,and can enhance the quality of life.
