STAT3对非小细胞肺癌诊断价值的探讨

摘　　要：目的探讨STAT3作为潜在肿瘤标志物对NSCLC的诊断价值。方法采用Real time PCR、ELISA方法,检测NSCLC患者、肺良性疾病患者、正常人血清中STAT3、CEA、CA125、CYFRA21-1蛋白表达水平;应用ROC曲线评价STAT3的诊断价值。结果肺癌组STAT3 mRNA、STAT3蛋白表达水平显著高于肺良性疾病组及正常对照组(P<0.01);两者的ROC曲线下面积(Az)分别为0.870和0.860,差异无统计学意义(P=0.500﹥0.05)。血清STAT3蛋白的Az分别与CEA、CA125、CYFRA21-1进行比较,差异均无统计学意义(P>0.05)。STAT3+CEA+CA125+CYFRA21-14项联检的Az为0.930,而CEA+CA125+CYFRA21-1 3项联检的Az为0.841;两者Az比较,差异有统计学意义(P<0.05)。结论 STAT3具有与CEA、CA125、CYFRA21-1对NSCLC同等的诊断价值及准确性,且其与另三者联合检测的诊断价值较高。Objective To explore the clinical diagnostic value of signal transducer and activator of transcription 3（STAT3） as a potential tumor marker.Methods Real time PCR and ELISA were applied to detect the expression of serum STAT3 protein in NSCLC patients、benign pulmonary disease patients and normal people.The ROC curve was used to analyze the diagnostic value of STAT3 protein in NSCLC.Results The expression levels of STAT3 mRNA and STAT3 protein in lung cancer group were significantly higher than that of the benign pulmonary disease group and normal control group（P0.01）.The area under ROC curves（Az） of STAT3 mRNA and STAT3 protein were 0.870、0.860 respectively,the Az between two groups had no significant difference（P0.05）.The Az of STAT3 protein had no significant difference in contrast to CEA 、CA125、CYFRA21-1（P0.05）.The Az of STAT3＋CEA ＋CA125＋CYFRA21-1 was 0.930,the Az of CEA ＋CA125＋CYFRA21-1 was 0.841,the Az of four joint detection was significantly larger than that of three joint detection（P0.05）.Conclusion The diagnostic value of STAT3 and CEA、CA125、CYFRA21-1 is equivalent respectively in NSCLC.The diagnostic accuracy and value of STAT3＋CEA ＋CA125＋CYFRA21-1 in NSCLC is significantly higher than that of CEA ＋CA125＋CYFRA21-1.
