基质金属蛋白酶2和缺氧诱导因子-1α在肝癌中的表达及意义

摘　　要：目的研究基质金属蛋白酶2(MMP-2)和缺氧诱导因子-1α(HIF-1α)在肝细胞癌中的表达及其临床意义。方法采用实时荧光定量PCR检测33例新鲜肝癌及相应癌旁组织标本中MMP-2和HIF-1αmRNA水平,应用免疫组化检测MMP-2、HIF-1α在45例肝癌及相应33例癌旁组织中的表达,分析其与临床病理特征间的关系以及MMP-2和HIF-1α表达的相关性。应用Kaplan-Meier生存分析研究MMP-2、HIF-1蛋白表达对患者生存时间的影响。结果实时荧光定量PCR结果表明肝癌组织中的MMP-2、HIF-1αmRNA水平分别为0.80±0.19、0.91±0.11,显著高于癌旁组织(0.68±0.15、0.65±0.19,P<0.05);免疫组化结果显示MMP-2、HIF-1α在肝癌组织中的表达率分别为62.22%和71.11%,显著高于癌旁组织(34.20%、36.8%,P<0.05),MMP-2和HIF-1α的表达与患者性别、年龄无关,而与肿瘤大小、转移、肿瘤包膜及临床TNM分期相关。MMP-2或HIF-1α阴性组患者的生存时间显著长于MMP-2或HIF-1α阳性组。结论 MMP-2、HIF-1α在肝癌中高表达,MMP-2信号通路可能和HIF-1信号通路共同促进了肝癌的进展。Objective To investigate the expression of MMP-2 and HIF-1α in hepatocellular carcinoma,and the relationship between MMP-2 or HIF-1α protein and the clinical or pathological changes in hepatocellular carcinoma.Methods The mRNA of MMP-2 and HIF-1α in 33 cases of hepatocellular carcinoma and its adjacent tissues were detected with real-time PCR,and the proteins of MMP-2 and HIF-1α in 45 cases of hepatocellular carcinoma and 33 cases of corresponding adjacent tissues were detected with immunohistochemical methods,and the relationship between MMP-2 or HIF-1α and pathologic features of hepatocellular carcinoma were analyzed.The correlation between MMP-2 and HIF-1α in hepatocellular carcinoma was also assessed.Kaplan-Meier methods was used to assess the impact of MMP-2 or HIF-1α expression on survival time.Results The relative quantification of MMP-2 and HIF-1α mRNA in hepatocellular carcinoma tissues was 0.80±0.19 and 0.91±0.11 respectively,which was significantly higher than that in adjacent tissues（0.68±0.15 and 0.65±0.19 respectively,P0.05）.The protein expression of MMP-2 and HIF-1α in hepatocellular carcinoma tissues was 62.22% and 71.11% respectively,significantly higher than that in adjacent tissues（34.20% and 36.8% respectively）.There was no significant correlation between the expression of MMP-2 or HIF-1α protein and the age,gender in patients with hepatocellular carcinoma（P0.05）,however there was significant correlation between the expression of MMP-2 or HIF-1α protein and tumor size,metastasis,capsule,and clinical TNM staging.MMP-2 and HIF-1α had a significant impact on survival time.Conclusions MMP-2 and HIF-1α overexpress in hepatocellular carcinoma and MMP-2 signaling pathway may promote development of the hepatocellular carcinoma with HIF-1α together.
