腹腔镜结肠癌根治术与开腹手术的临床疗效对比研究

摘　　要：目的比较腹腔镜与开腹手术治疗结肠癌的临床疗效及术后初期生活质量差异。方法选择我院2008年3月—2011年2月治疗的结肠癌患者79例,其中腹腔镜组42例,开腹组37例,比较两组的术中情况和术后1周、4周时的生活质量。结果腹腔镜组在术中出血量、术后肠功能恢复时间、术后下床活动时间、恢复工作时间、住院总天数等方面优于开腹组,而手术时间长于开腹组、总住院费方面略高于开腹组,差异均有统计学意义(P<0.05);腹腔镜组患者术后1周疼痛情况优于开腹组,差异有统计学意义(P<0.05);而术后生活质量测定核心量表(QLQ-C30)其他领域评分比较,差异均无统计学意义(P>0.05)。结论腹腔镜结肠癌根治术具有创伤小、恢复快,术后初期疼痛轻等优点,值得临床推广应用。Objective To compare the clinical effectiveness and short - term postoperative quality of life between lapa- roscopic and open radical operation for colon cancer. Methods Totally 79 patients with colon cancer who were treated in our hospital from March 2008 to April 2011 were divided into laparoscopic group （ n = 42 ） and open group （ n = 37 ）. Both groupsreceived radical operation. The intraoperative indicators and quality of life one week and four weeks after operation were compared between these two groups. Results The intraoperative blood loss, recovery of gastroinestinal function, ambulatory time, esti- mated time to resume work, and hospital stay were significantly superior in the laparoscopic group than in the open group, al-though the laparoscopic group had slightly longer operation duration and hospitalization expense （ allP 〈 0. 05）. Pain in the first week after operation was significantly better in the laparoscopic group than in the open group （P 〈 0. 05 ）. Post - operative quali- ty of life and other indicators showed no significant difference between these two groups （P 〉 0. 05）. Conclusion Laparoscopicradical operation has advantages including small trauma, quick recovery, and mild postoperative pain, and therefore is feasible for treating colon cancer.
