重症乳磨牙根尖周炎症治疗的长期观察

摘　　要：目的寻找适合国情、简单有效的治疗重症乳磨牙根尖周炎的方法,以达到保存乳牙、减少继承恒牙疾患发生的目的。方法 22例因重症根尖周炎造成根方骨质吸收并波及牙胚的乳磨牙,采用Vitapex糊剂进行根管治疗,临床长期观察至继承恒牙牙冠完全萌出建合。结果 1例短期内治疗失败,6例失访,15例萌出建合位置正常,其中1例唇侧面有少于1mm×1.5mm面积的黄斑。结论根尖周炎症造成根方骨质吸收并波及继承牙胚做为拔除乳牙的绝对指征值得商榷,结合乳牙的临床状况、继承牙胚钙化度等方面进行适当的病例选择,采用根管治疗术后对继承恒牙的发育影响轻微。OBJECTIVE To find a therapy of serious periodical diseases of primary teeth,which is simple and fit the reality of China,so as to reserve the primary teeth involved and reduce the possibility of disease happened on successor permanent teeth.METHODS 22 patients with the Vitapex paste got the root canal therapy,and they were observed in clinics until the completely come out of dental crown of permanent teeth.RESULTS Only 1 case failed after the therapy,and 6 cases were lost.The periodical diseases of 15 primary molars had been well controlled after the root canal therapy,established the occlusal at proper places.One case had yellow spot of no more than 1 mm × 1.5 mm on Labial side.CONCLUSION It is not appropriate to take it as an only standard that to extract a primary molar just because of its periapical disease involved the tooth germ below.Whether to extract such a primary molar should consider the age of patient,clinical syndrome,calcifying of the tooth germ below and so on,and then evaluate all the situation in detail.It is not necessary to remove the primary molar.
